清热解毒通络汤联合膝痹汤外洗治疗重度膝骨关节炎疗效观察

Observation on curativeeffect of qingre jiedu tongluo decoction combined with xiebi decoction external washing in treatment ofsevere knee osteoarthritis

目的探讨清热解毒通络汤联合膝痹汤外洗治疗重度膝骨关节炎疗效,为临床治疗重度膝骨关节炎提供依据。方法本次研究纳入100例重度膝关节炎患者,均由我院2017年5月~2018年9月收治,以随机数字表法将其分为观察组和对照组,对照组患者(50例)实施人工全膝关节置换术治疗,观察组患者(50例)在对照组治疗基础上联合清热解毒通络汤及膝痹汤外洗治疗,比较两组患者疗效、治疗前后患者症状积分、治疗前后HSS髋关节功能评分及生活质量评分(SF-36量表评分)、并发症情况。结果观察组患者临床治疗有效率高于对照组,差异有统计学意义(P <0.05);治疗前,两组患者症状积分及HSS评分、SF-36量表评分相当,差异无统计学意义(P> 0.05),治疗后各组患者症状积分及HSS评分、SF-36量表评分均改善,观察组患者症状积分及HSS评分、SF-36量表评分优于对照组,差异有统计学意义(P <0.05);观察组患者并发症发生率低于对照组,差异有统计学意义(P <0.05)。结论清热解毒通络汤联合膝痹汤外洗治疗重度膝骨关节炎临床疗效显著,且患者症状改善,髋关节功能改善,生活质量提升,患者并发症率低,安全性高,值得临床以及应用。

Objective To explore the curative effect of Qingre Jiedu Tongluo Decoction combined with Xiebi Decoction external washing in treatment of severe knee osteoarthritis. Methods 100 patients with severe knee osteoarthritis who were treated in our hospital from May 2017 to September 2018 were included in this study.They were divided into observation group and control group by random number table method.The control group(50 cases) was given total knee replacement while the observation group(50 cases) was treated by Qingre Jiedu Tongluo Decoction combined with Xiebi Decoction xternal washing on the basis of the control group.The curative effect,symptom score before and after treatment,HSS hip joint function score before and after treatment,quality of life score(SF-36 scale score) before and after treatment and complications of the two groups were compared. Results The effective rate of clinical treatment in the observation group was higher than that in the control group(P < 0.05).Before treatment,the scores of symptoms,HSS and SF-36 scale in the two groups were similar(P > 0.05).After treatment,the scores of symptoms,HSS and SF-36 scale in every group were improved.The scores of symptoms,HSS and SF-36 scale in the observation group were better than those in the control group,(P < 0.05).The incidence of complications in the observation group was lower than that in the control group(P < 0.05). Conclusion The clinical effect of Qingre Jiedu Tongluo Decoction combined with Xiebi Decoction external washing in treatment of severe knee osteoarthritis was significant.It can improve symptoms of patients,hip joint function and quality of life and reduce complication rate.In addition,it has high safety,which is worthy of clinical application.
