双钢板固定植骨术结合经皮复位有限固定术在SchatzkerⅤ/Ⅵ型胫骨平台骨折中的应用

Application of double plate fixation combined with percutaneous reduction and limited fixation in Schatzker Ⅴ/Ⅵ tibial plateau fracture

目的给予SchatzkerⅤ/Ⅵ型胫骨平台骨折患者实施双钢板固定植骨术结合经皮复位有限固定术治疗,并对其治疗效果进行分析。方法选取我院2016年7月~2017年7月收治的50例SchatzkerⅤ/Ⅵ型胫骨平台骨折患者,随机将其分为对照组与实验组,对照组25例患者给予双板固定植骨术进行治疗,实验组25例患者给予双钢板固定植骨术结合经皮复位有限固定术进行治疗,对比其治疗效果。结果 (1)实验组患者膝关节功能优良率(92.0%)较对照组患者膝关节功能优良率(68.0%)高,差异有统计学意义(P <0.05)。(2)两组患者手术时间比较并无统计学差异(P> 0.05);实验组患者术中出血量与愈合时间均低于对照组患者,差异有统计学意义(P <0.05)。结论 SchatzkerⅤ/Ⅵ型胫骨平台骨折采取双钢板固定植骨术结合经皮复位有限固定术进行治疗有效的降低了对膝关节周围软组织造成的伤害,加速了膝关节功能的恢复,且具有较高的安全性,值得推广使用。

Objective To treat Schatzker Ⅴ/Ⅵ tibial plateau fracture with double plate fixation combined with percutaneous reduction and limited fixation and to analyze its therapeutic effect. Methods 50 patients with SchatzkerⅤ/Ⅵ tibial plateau fracture who were admitted and treated in our hospital from July 2016 to July 2017 swere selected and they were randomly divided into the control group and the experimental group. 25 patients in the control group were treated with double plate fixation while 25 patients in the experimental group were treated with double plate fixation combined with percutaneous reduction and limited fixation. Therapeutic effects were compared. Results(1)The excellent and good rate of knee joint function in the experimental group was 92.0%, higher than that in the control group 68.0%. There was significant difference in data(P < 0.05).(2)There was no difference in the operative time between the two groups(P > 0.05). The intraoperative blood loss and healing time in the experimental group were both lower than those in the control group. There was significant difference in data(P < 0.05). Conclusion Double plate fixation combined with percutaneous reduction and limited fixation in treatment of Schatzker Ⅴ/Ⅵ tibial plateau fracture can effectively reduce the damage to the soft tissue around the knee joint and accelerate the recovery of knee joint function. And it has high safety, which is worthy of promotion and application.
