综合康复干预措施在手部指屈肌腱损伤治疗中的应用效果

Application effect of comprehensive rehabilitation intervention in the treatment of flexor tendon injury of the hand

目的探讨综合康复干预措施在手部指屈肌腱损伤治疗中的应用效果。方法将2015年1月~2017年12月我院收治的106例手部指屈肌腱损伤患者作为研究对象,随机分为两组,各53例。两组患者均改良Kessler缝合修补术。对照组给予常规康复方法,观察组患者给予综合康复干预措施。比较两组患者临床疗效、手术住院相关指标、生活质量及粘连发生率的差异。结果观察组患者显效率、总有效率分别为60.3%、90.6%显著高于对照组30.2%、66.0%,差异有统计学意义(P <0.05);观察组患者TAM评分显著高于对照组,其住院天数及疼痛评分显著低于对照组,差异有统计学意义(P <0.05);手术后观察组患者生活质量评分显著升高,其也高于同期对照组,差异有统计学意义(P <0.05);观察组患者术后粘连发生率为5.7%显著低于对照组26.4%,差异有统计学意义(P <0.05)。结论综合康复干预措施对手部指屈肌腱损伤治疗效果佳,其临床有效率较高,疼痛少、恢复快,并发症也较差,生活质量较高,值得临床选择。

Objective To explore the application effect of comprehensive rehabilitation intervention in the treatment of flexor tendon injury of the hand. Methods 106 patients with flexor tendon injury of the hand who were admitted and treated in our hospital from January 2015 to December 2017 were selected as research objects.They were randomly divided into two groups,with 53 cases in each group.Patients in both groups were given modified Kessler suture repair.The control group was given conventional rehabilitation methods while the observation group was given comprehensive rehabilitation intervention measures.The clinical curative effects,surgical hospitalization related indexes,quality of life and incidence of adhesion were compared between two groups. Results The obvious effective rate and overall effective rate in the observation group were respectively 60.3% and 90.6%, higher than 30.2% and 66.0% in the control group.The differences were statistically significant(P < 0.05).AM scores of patients in the observation group were significantly higher than those in the control group,and their hospital stay and pain scores were significantly lower than those in the control group.The differences were statistically significant(P < 0.05).The quality of life score of the observation group was significantly increased after surgery, which was also higher than that of the control group.The differences were statistically significant(P < 0.05).The postoperative adhesion rate was 5.7% in the observation group and 26.4% in the control group.The difference was statistically significant(P < 0.05). Conclusion Comprehensive rehabilitation intervention measures have good curative effects in the treatment of flexor tendon injury of the hand,with higher clinical effective rates,less pain,faster recovery,poor complications and higher quality of life,which is worthy of clinical selection.
