单眼直肌手术治疗成人知觉性外斜视的临床分析

Clinical Analysis of Single-ocular Rectus Surgery for Adult Perceptual Exotropia

目的观察和分析单眼直肌手术治疗成人知觉性外斜视的临床效果。方法该课题选取的48例成人知觉性外斜视患者均是该院2017年11月—2018年11月收治,由单双病号分组,A组24例患者实行单眼直肌超常量手术治疗,B组24例患者实行单眼直肌常量手术治疗,对比两组手术效果。结果 A组患者术眼正位率为91.67%,高于B组79.14%,而A组患者眼球运动正常率为87.50%,低于B组91.67%,两组手术效果对比差异无统计学意义(P>0.05),所有患者术后均未出现并发症。结论单眼直肌手术治疗成人知觉性外斜视的临床效果较显著,能有效改善患者知觉性外斜视症状,且手术安全性较高,但单眼直肌超常量手术正位效果更好,但极易出现眼球外展不足缺陷,而单眼直肌常量手术眼球运动效果好,但正位效果比超常量差,故临床上需要根据患者实际病情合理选择手术方案。

Objective To observe and analyze the clinical effect of single-ocular rectus surgery in the treatment of adult perceptual exotropia. Methods 48 adult patients with perceptual exotropia were enrolled in our hospital from November 2017 to November 2018. They were divided into single and double disease groups. Group A of 24 patients underwent monocular rectus hyperthermia surgery. Group B of 24 patients underwent monocular rectus major surgery,comparing the effects of the two groups. Results The orthotopic rate of group A was 91.67%, which was higher than that of group B(79.14%). The normal rate of eye movement in group A was 87.50%, which was lower than that in group B of 91.67%. The difference between the two groups was no significant difference(P>0.05), all patients had no complications after surgery. Conclusion The clinical effect of single-ocular rectus surgery on adult perceptual exotropia is significant, which can effectively improve the patient’s perceptual exotropia symptoms, and the operation safety is higher, but the single-eye rectus hyperthermia surgery is better, but it is very easy. There is a defect in eyeball abduction, and the eye movement of the monocular rectus is excellent, but the orthotopic effect is worse than the superconstant. Therefore, it is necessary to rationally select the surgical plan according to the actual condition of the patient.
