全面护理干预对儿童糖尿病酮症酸中毒的影响

Effect of Comprehensive Nursing Intervention on Diabetic Ketoacidosis in Children

目的研究全面护理干预对儿童糖尿病酮症酸中毒的影响。方法随机从医院收治的儿童糖尿病酮症酸中毒患儿中抽取30例作为研究对象,将其分成参照组和观察组两组,每组15例患儿。对参照组实施一般性护理管理,对观察组实施全面护理干预,比较患儿的住院费用、住院时间、血糖控制情况。结果观察组患儿平均住院费用(5825.6±526.8)元、平均住院时间(10.2±1.1)d,明显少于参照组的平均住院费用(6 925.4±445.1)元、平均住院时间(14.3±2.8)d;两组患儿入院时糖化血红蛋白值无比较差异,观察组出院后3个月的糖化血红蛋白值(8.1±0.8)%,显著低于参照组的(10.6±1.3)%。观察组低血糖发生率6.7%,低于参照组的26.7%,患儿pH值、尿酮及餐后血糖值达标时间均明显早于参照组。差异有统计学意义(P<0.05)。结论全面护理干预对儿童糖尿病酮症酸中毒治疗具有积极影响,临床应用价值高,值得推广。

Objective To study the effect of comprehensive nursing intervention on diabetic ketoacidosis in children. Methods Thirty children with diabetic ketoacidosis admitted to the hospital were randomly selected as study subjects. They were divided into two groups: reference group and observation group, with 15 children in each group. General nursing management was implemented for the reference group, and comprehensive nursing intervention was implemented for the observation group to compare the hospitalization expenses, hospitalization time, and blood glucose control of the child. Results The average hospitalization cost (5 825.6±526.8) yuan and the average hospitalization time (10.2±1.1) d were significantly lower in the observation group than in the reference group (6 925.4±445.1) yuan and the average length of hospital stay (14.3±2.8) d;There was no significant difference in glycated hemoglobin values between the two groups at admission. The glycosylated hemoglobin value of the observation group was (8.1±0.8)% of three months after discharge, which was significantly lower than that of the reference group (10.6±1.3)%. The incidence of hypoglycemia in the observation group was 6.7%, which was lower than 26.7% in the reference group. The PH value, urine ketone and postprandial blood glucose level of the observation group were significantly earlier than the reference group, statistically significant(P<0.05). Conclusion Comprehensive nursing intervention has a positive effect on the treatment of diabetic ketoacidosis in children. It has high clinical value and is worthy of promotion.
