经阴道三维能量多普勒超声及磁共振成像对子宫内膜癌肌层浸润判断的对比研究

Comparative study of transvaginal three-dimensional power Doppler ultrasound and MRI in the diagnosis of endometrial cancer myometrial invasion

目的比较经阴道三维能量多普勒超声及磁共振成像(magnetic resonance imaging,MRI)对子宫内膜癌肌层浸润的判断价值。方法回顾性分析2016年7月至2018年1月在甘肃省妇幼保健院接受子宫内膜癌手术治疗72例患者的临床资料,患者术前行经阴道三维超声及MRI检查,比较两种方法诊断子宫内膜癌肌层浸润的敏感性、特异性、阳性预测值、阴性预测值及准确率。结果经阴道三维能量多普勒超声和MRI的敏感性、特异性、阳性预测值、阴性预测值及准确率分别为78.3%、76.9%、78.3%、76.9%、77.8%及82.2%、81.5%、82.2%、81.5%、81.9%;ROC曲线分析显示,经阴道三维能量多普勒超声和MRI曲线下面积分别为0.844和0.885,差异有统计学意义(P<0.05)。结论经阴道三维能量多普勒超声及MRI可以预测子宫内膜癌肌层浸润,有很好的临床应用价值。

Objective The value of transvaginal three-dimensional power Doppler ultrasound and MRI in the evaluation of myometrial invasion of endometrial cancer was compared.Methods Retrospective analyzed the clinical data of 72 patients undergoing endometrial cancer surgery in Gansu Provincial Maternity and Child-care Hospital from July 2016 to January 2018.The patients underwent transvaginal three-dimensional ultrasound and MRI before operation.The sensitivity,specificity,positive predictive value,negative predictive value and accuracy of the two methods for diagnosing myometrial invasion of endometrial cancer were compared.Results The sensitivity,specificity,positive predictive value,negative predictive value and accuracy of transvaginal three-dimensional power Doppler ultrasound and MRI were 78.3%,76.9%,78.3%,76.9%,77.8%and 82.2%,81.5%,82.2%,81.5%,81.9%,respectively;ROC curve analysis showed that the area under transvaginal three-dimensional power Doppler ultrasound and MRI curve were 0.844 and 0.885,respectively,and the difference was statistically significant(P<0.05).Conclusion Transvaginal three-dimensional power Doppler ultrasound and MRI can predict myometrial invasion of endometrial cancer,which has good clinical application value.
