超选择插管在肝癌介入治疗中对患者血清AFP、肝功能的影响及应用价值

Effect of Superselective Intubation on Serum AFP and Liver Function in Patients with Hepatocellular Carcinoma Intervention and Its Clinical Value

目的探讨超选择插管在肝癌介入治疗中对患者血清AFP、肝功能的影响。方法选择122例肝癌患者,根据治疗方式不同分为实验组和对照组各61例,实验组行超选择插管介入治疗,对照组行常规选择插管治疗,对比2组治疗后的总有效率、患者血清AFP水平以及肝功能指标AST、ALT、TBIL等。结果经过治疗后,实验组总有效率为96. 72%(59/61)显著高于对照组83. 61%(51/61),差异有统计学意义(χ~2=5. 92,P <0. 05);治疗后两组患者的血清甲胎蛋白(alpha-fetoprotein,AFP)水平显著优于治疗前,且实验组血清AFP水平变化大于对照组(t=9. 79,P <0. 05),差异有统计学意义;治疗后实验组患者的谷草转氨酶(AST)、谷丙转氨酶(ALT)总胆红素(TBIL)等肝功能指标波动均小于对照组(t=5. 74、3. 28、3. 39,P <0. 05),差异有统计学意义。结论肝癌介入治疗采用超选择插管可以有效提高治愈率,改善病情,改善血清AFP水平,对肝功能损伤小,改善预后,值得临床应用推广。

Objective To investigate the effect of superselective intubation on serum AFP and liver function in interventional treatment of liver cancer. Methods A total of 122 patients with hepatocellular carcinoma were divided into experimental group and control group,61 cases in each group according to different treatment methods. The experimental group was treated with super-selective intubation and the control group was treated with routine intubation. The total effective rate,serum AFP level and liver function indexes AST,ALT,TBIL were compared after treatment between the two groups. Results After treatment,the total effective rate in the experimental group was 96. 72%( 59/61) was significantly higher than the control group 83. 61%( 51/61),the difference was statistically significant(χ^2= 5. 92);The level of serum alpha-fetoprotein( AFP) in the group was significantly better than before treatment,and the level of serum AFP in the experimental group was significantly greater than that in the control group( t = 9. 79). The difference was statistically significant;The fluctuations of liver function indexes such as Aspartate aminotransferase( AST) and Alanine aminotransferase( ALT) total bilirubin( TBIL) were lower in the group than in the control group( t = 5. 74,3. 28,3. 39),the difference was statistically significant. Conclusion The use of superselective intubation in the interventional treatment of liver cancer can effectively improve the cure rate,improve the condition,improve the level of serum AFP,have less damage to the liver function,and improve the prognosis,which is worthy of clinical application.
