超声引导微创切除术治疗乳腺良性与恶性肿块的效果及安全性比较

Compressive of the Efficacy and Safety of Ultrasound-guided Minimally Invasive Resection for Benign and Malignant Breast Tumors

目的对超声引导微创切除术治疗乳腺良性与恶性肿块中的效果及安全性进行比较。方法选取乳腺良恶性肿块患者200例纳入研究,其中良性肿块患者和恶性肿块患者分别为120例和80例。对比患者病理特征,并且对于超声引导微创切除治疗的效果以及安全性进行对比评价。结果两组患者基本特征未见差别,P> 0. 05。年龄分布、肿块特征分布充分可比。同时可见恶性肿物患者中的乳腺癌家族史比例高于良性肿物组,差别有统计学意义,P <0. 05。80例恶性肿物患者中78例随后的24 h内接受了乳腺切除手术。术中沿穿刺路线切取病理,未见肿瘤细胞扩散,提示安全性良好。此外2组患者总体满意率均高,分别为94. 17%和93. 75%;除了恶性肿物组术中出血率较高(12. 50%),且同时高于良性肿物组(P <0. 05)之外;术中和术后各种并发症发生率均较低,安全性良好,提示本微创术式值得临床推广。结论超声引导的微创术式用于治疗良性乳房肿物满意率高,安全性好;对于恶性肿物切除也具有高满意度和较好的安全性,且为后续全切手术打下良好基础,值得临床推广。

Objective To compare the efficacy and safety of ultrasound guided minimally invasive resection in the treatment of benign and malignant breast masses. Methods A total of 200 patients with breast benign and malignant tumors were selected from our hospital. Among them,120 patients and 80 patients with benign tumors and malignant tumors were included. The pathological features of the patients were compared with the comparative evaluation of the efficacy and safety of ultrasound-guided minimally invasive resection. Results There was no difference in basic characteristics between the two groups of patients in this study,P > 0. 05. The age distribution and mass distribution of the mass are fully comparable. At the same time,the proportion of family history of breast cancer in patients with malignant tumors was higher than that in benign tumors group,with a statistically significant difference( P < 0. 05). Among the 80 patients with malignant tumors,78 were followed by mastectomy within 24 hours.During the operation,pathology was taken along the puncture route. No tumor cell proliferation was observed,suggesting good safety. In addition,the overall satisfaction rate was high in the two groups,which were 94. 17% and 93. 75%,respectively;except for the malignant tumor group,the intraoperative bleeding rate was higher( 12. 50%),and was also higher than that of the benign tumor group( P < 0. 05);The incidence of various complications during and after surgery was low and safety was good,suggesting that the minimally invasive surgery is worthy of clinical promotion. Conclusion Ultrasound-guided minimally invasive surgery for the treatment of benign breast tumors has high satisfaction rate and good safety. It also has high satisfaction and good safety for malignant tumor resection and lays a good foundation for follow-up surgery. It is worthy of clinical promotion.
