运用SBAR模式,探索护理人员在危重患者床头交接班的实践 被引量:4

摘要 目的通过开展品管圈,探讨运用SBAR沟通模式提高护理人员对危重患者床头交接班质量的应用效果。方法 2017年3-6月开展全院范围的提高危重患者床头交接班质量的品管圈活动,成立专项工作质量改善小组,制定运用SBAR沟通模式的床头交接班流程和关键内容查核表,查核危重患者床头交接班132例。比较品管圈活动前后危重患者床头交接班交接内容的缺陷率、责任护士对分管患者有效信息("十知道")的知晓度及主管医生和患者对护士工作的满意度。结果SBAR沟通模式运用到危重患者床头交接班的品管圈活动后,护士交接班缺陷率由18.28%下降到3.32%;责任护士对患者有效信息知晓度、医生和患者对护士工作的满意率较前提高,差异均有统计学意义(P<0.05)。结论通过开展在危重患者床头交接班中运用SBAR沟通模式的品管改善活动,规范了床头交接班流程,有效提高护理人员交接班的效果与质量,提升了护士对危重症患者的护理能力和医生、患者的满意度。 Objective By launching QCC(quality control circle), to investigate the application effect of utilizing the SBAR communication mode to improve the nursing bedside shift quality on critically ill patients.Methods QCC activity was launched in the hospital-wide from March to June in 2017, aiming to improve the bedside shift quality on critically ill patients. A specialized work group was set up on quality improvement. The bedside shift process and key content checklist is established, utilizing the SBAR communication mode. 132 cases of bedside shift on critically ill patients were examined. The defect rate of bedside shift content, the nurse awareness of patient’s effective information(TEN AWARENESS) and the satisfaction rate of doctor and patient on the work of the nurse before and after launching QCC activity was compared. Results After launching QCC activity in nursing bedside shift on critically ill patients, the defect rate of the nursing shift was lowered from 18.28% to 3.32%, the nurse awareness of patient’s effective information and the satisfaction rate of doctor and patient on the work of the nurse both improved. The differences were meaningful in statistics(P < 0.05). Conclusion By launching QCC activity in nursing bedside shift quality on critically ill patients, the bedside shift process is standardized, the nurse shift effectiveness and quality are well improved, nursing quality on critically ill patients and satisfaction rate of patients and doctors are all improved.
