摘要
目的研究NSE、ProGRP、SCC-Ag、CYFRA21-1、CEA和CA1256种肿瘤标志物在血清中表达分布及联合检测对肺癌辅助诊断和病理分型的应用价值。方法应用AutolumoA2000检测314例肺癌患者和364例肺良性疾病患者血清中的6种肿瘤标志物的表达水平,分析联合检测对辅助肺癌病理分型的应用价值。结果肺癌患者组血清中6种肿瘤标志物的检测结果明显高于肺良性病变组,差异有统计学意义(P<0.01);NSE和ProGRP在小细胞肺癌患者血清中的阳性率为78.55%;非小细胞肺癌病变组中,SCCA和CYFRA21-1在肺鳞癌患者组血清中的阳性率为82.46%,CEA和CA125在肺腺癌患者中的阳性率为57.57%。结论联合检测NSE、ProGRP、SCC-Ag、CYFRA21-1、CEA和CA125可明显提高肺癌的检出率;6种标志物联合检测对肺癌的病理分型有一定的参考意义,NSE+ProGRP联合检测对小细胞肺癌的敏感度更高;非小细胞肺癌组中,SCC-Ag+CYFRA21-1联合检测对肺鳞癌的敏感度更高;CEA+CA125联合检测对肺腺癌的敏感度更高。
Objective To explore the expression, distribution and the combined detection of six tumor markers ( CEA, CA125, NSE, ProGRP, SCC - Ag, CYFRA21 - 1) in serum and their application value in auxiliary diagnosis and pathological classification of lung cancer. Methods Autolumo A2000 was used to detect the expression of six tumor markers in serum of 314 patients with lung cancer and 364 patients with benign lung diseases, and the value of combined detection in pathologic classification of lung cancer was analyzed. Results The expression of 6 tumor markers in patients with lung cancer was significantly higher than that in patients with benign diseases ( P < 0. 01);The positive rate of NSE and ProGRP in serum of patients with small cell lung cancer was 78. 55%. The positive rate of SCCA and CYFRA21 - 1 in patients with lung squamous cell carcinoma was 82. 46% and the positive rate of CA125 in lung adenocarcinoma was 57. 57% in patients with non - small cell carcinoma. Conclusion The combined detection of CEA, CA125, ProGRP, SCC - Ag, CYFRA21 - 1 can significantly improve the detection rate of lung cancer. The combined detection of six markers has certain reference significance for the pathological classification of lung cancer. The combined detection of NSE and ProGRP is more sensitive to small cell lung cancer;the combined detection of SCC - Ag and CYFRA21 - 1 is more sensitive to lung squamous cell carcinoma;and the combined detection of CEA and CA125 is more sensitive to lung adenocarcinoma.
