锥颅血肿碎吸术成功救治大脑中动脉支架植入术后高灌注脑出血患者一例报道

Successful Treatment of Hyperperfusion-induced Cerebral Hemorrhage after Middle Cerebral Artery Stenting by Transcranial Drilling Hematoma Aspiration:a Case Report

高灌注损伤综合征(HS)是脑血管支架植入术后严重并发症之一,主要临床表现为头痛、癫痫发作、脑水肿,严重者甚至出现脑出血。目前,高灌注脑出血仍是困扰神经介入医生的一大难题,临床尚无相关指南及规范指导其治疗,因此患者病死率较高。笔者所在医院结合临床经验采用锥颅血肿碎吸术成功救治一例大脑中动脉支架植入术后高灌注脑出血患者并首次进行报道,以期为提高临床高灌注脑出血救治水平提供参考。

Hyperperfusion syndrome is one of serious complications secondary to middle cerebral artery stenting,which is mainly manifested as headache,epileptic seizure,brain edema,and even cerebral hemorrhage. At present,hyperperfusion-induced cerebral hemorrhage is still a great challenge for neurointerventionist.Due to lack of corresponding treatment guidelines,thus lead to a high fatality rate. Based on clinical experience,we successfully treated a patient with hyperperfusion-induced cerebral hemorrhage after middle cerebral artery stenting by transcranial drilling hematoma aspiration and specially reported it for the first time,to provide a reference for improving clinical treatment.
