利拉鲁肽联合盐酸二甲双胍片治疗2型糖尿病的临床效果及对患者治疗依从性的影响

Clinical effect of liraglutide combined with metformin hydrochloride tablets in the treatment of type 2 diabetes mellitus and its influence on patients’ compliance

目的研究利拉鲁肽联合盐酸二甲双胍片治疗2型糖尿病(T2DM)的临床效果及对患者治疗依从性的影响。方法将我院2017年9月至2018年6月收治的62例T2DM患者随机分为对照组和观察组,各31例。对照组给予盐酸二甲双胍片治疗,观察组在此基础上给予利拉鲁肽治疗,两组均连续治疗12周。比较两组患者治疗前、后血糖指标、内脂素、瘦素、胰岛功能及血脂水平,记录两组患者治疗依从性及不良反应发生情况。结果治疗后,两组患者FPG、2hPG、HbA1c、内脂素和瘦素水平均显著降低,且观察组均显著低于对照组(P<0.05)。治疗后,观察组空腹胰岛素水平显著升高,两组HOMA-IR显著降低,HOMA-β显著升高,且观察组优于对照组(P<0.05)。治疗后,两组血清TC、TG及LDL-C水平均显著降低,HDL-C水平显著升高,且观察组均优于对照组(P<0.05)。两组患者治疗依从性及各项不良反应发生率比较,差异均无统计学意义(P>0.05)。结论利拉鲁肽联合盐酸二甲双胍片能够有效控制T2DM患者血糖水平,改善胰岛功能,降低血脂水平,且不影响患者治疗依从性,安全性较高,值得临床推广使用。

Objective To study the clinical effect of liraglutide combined with metformin hydrochloride tablets in the treatment of type 2 diabetes mellitus (T2DM) and its influence on patients' compliance.Methods A total of 62 patients with T2DM admitted in our hospital from September 2017 to June 2018 were randomly divided into control group and observation group, with 31 cases in each group.The control group was treated with metformin hydrochloride tablets and the observation group was treated with liraglutide on this basis.Both groups were treated continuously for 12 weeks.The levels of blood sugar indexes, visfatin, leptin, islet function and blood lipid before and after treatment were compared between the two groups;the treatment compliance and incidence of adverse reactions of the two groups were recorded.Results After treatment, the levels of FPG, 2 h PG, HbA1c, visfatin and leptin in the two groups decreased significantly, and those in the observation group were significantly lower than the control group (P<0.05).After treatment, the fasting insulin level in the observation group increased significantly, HOMA-IR decreased significantly and HOMA-β increased significantly in the two groups, and those in the observation group were better than the control group(P<0.05).After treatment, the serum levels of TC, TG and LDL-C of the two groups decreased significantly, while the levels of HDL-C increased significantly, and those in the observation group were better than the control group (P<0.05).There were no significant differences in the treatment compliance and incidences of adverse reactions of patients between the two groups (P>0.05).Conclusion Liraglutide combined with metformin hydrochloride tablets can effectively control the blood sugar level of patients with T2DM, improve the function of islets, reduce the levels of blood lipids.It does not affect the treatment compliance of patients and with a high safety, which is worthy of clinical promotion and application.
