乳腺癌原发灶超声特征与腋窝淋巴结转移的关系 被引量:20

摘要 目的:探讨乳腺癌原发灶超声特征与腋窝淋巴结转移的关系。方法:回顾性选取2015年6月至2017年6月本院收治的乳腺癌患者112例,所有患者均给予超声检查,并以术后病理结果为对照,分析乳腺癌原发灶超声特征与腋窝淋巴结转移的关系。结果:本研究中,术后病理结果显示腋窝淋巴结转移52例(46.43%);单因素分析结果显示,肿瘤病理类型、肿瘤最大直径、毛刺征、钙化、血流分级、腋窝淋巴结数与腋窝淋巴结转移有关(P<0.05);Logistic回归性分析结果显示,校正肿瘤病理类型后,肿瘤最大直径>2cm、毛刺征、钙化、血流Ⅲ~Ⅳ级、腋窝淋巴结数>2个是腋窝淋巴结转移的独立危险因素(P<0.05)。结论:乳腺癌原发灶超声特征与腋窝淋巴结转移有关,肿瘤最大直径>2cm、毛刺征、钙化、血流Ⅲ~Ⅳ级、腋窝淋巴结数>2个对评估腋窝淋巴结转移具有重要的提示作用,值得临床作进一步推广。 Objective: To discuss the relationship between the ultrasonographic features of primary breast cancer and the axillary lymph node metastasis. Methods: 112 patients with breast cancer were selected retrospectively from June 2015 to June 2017 in our hospital,they were examined by ultrasonography,and the postoperative pathological findings were taken as the control.The relationship between the ultrasonographic characteristics of primary breast cancer and the axillary lymph node metastasis was analyzed. Results: In this study,the postoperative pathological results showed that 52 cases of axillary lymph node metastasis (46.43%).Single factor analysis showed that the tumor pathological type,the maximum diameter of the tumor,spicule sign,calcification,blood flow classification and axillary lymph node number were associated with axillary lymph node metastasis ( P <0.05).Logistic regression analysis showed that,the maximum diameter >2 cm,spicule sign,calcification,grade Ⅲ-Ⅳ,and >2 of axillary lymph nodes were the independent risk factors of axillary lymph node metastasis( P <0.05). Conclusion: The ultrasonographic features of primary breast cancer are associated with axillary lymph node metastasis,the maximum diameter of >2 cm,spicule sign,calcification,grade Ⅲ-Ⅳ and >2 in axillary lymph nodes have important implications for the evaluation of axillary lymph node metastasis.It′s worth for further clinical promotion.
