经阴道超声评价曼月乐治疗子宫内膜增生过长的临床研究

Clinical Study on Transvaginal Ultrasound in Evaluating Mirena in the Treatment of Endometrial Hyperplasia

目的探讨经阴道超声在曼月乐(左炔诺孕酮宫内缓释系统)治疗子宫内膜增生过长效果评价中的价值。方法选取2015年3月至2018年2月我院收治的53例子宫内膜增生过长患者,用经阴道超声测量曼月乐放置前及放置6个月后的子宫内膜厚度、子宫动脉阻力指数及搏动指数。结果 53例子宫内膜增生过长患者中, 49例患者内膜转归正常,治疗成功率为92.45%。放置6个月后,患者的子宫内膜厚度、子宫动脉阻力指数及搏动指数均明显低于放置前(P <0.05)。结论经阴道超声可较好地评价曼月乐对子宫内膜增生过长的临床治疗效果,尤其是子宫动脉频谱能够直观有效地反映子宫内膜变化,临床应用价值较高。

Objective To explore the value of transvaginal ultrasound in effect evaluation of mirena(levonorgestrel releasing intrauterine system) in the treatment of endometrial hyperplasia. Methods 53 cases of patients with endometrial hyperplasia admitted to our hospital from March 2015 to February 2018 were selected. The endometrium thickness, uterine artery resistance index and pulsatility index before and 6 months after mirena placement were measured by transvaginal ultrasound. Results Among 53 cases of patients with endometrial hyperplasia, 49 cases had normal endometrium, with the treatment success rate of 92.45%. 6 months after placement, the endometrial thickness, uterine artery resistance index and pulsatility index were significantly lower than those before placement(P<0.05).Conclusions Transvaginal ultrasound can be better used to evaluate the clinical treatment effect of mirena on endometrial hyperplasia,especially the uterine artery frequency spectrum can reflect the changes of endometrium directly and effectively, with higher clinical application value.
