不同数量空心钉内固定治疗股骨颈骨折的比较研究

Comparative Study on Internal Fixation with Different Numbers of Cannulated Screws in the Treatment of Femoral Neck Fracture

目的探讨不同数量空心钉内固定治疗股骨颈骨折的疗效。方法选取我院2016年1月至2017年11月行空心钉内固定治疗的58例老年股骨颈骨折患者,按不同数量空心钉内固定方式分为观察组(4枚空心钉内固定)与对照组(3枚空心钉内固定)各29例,比较两组的治疗效果。结果术前、术后3个月、术后9个月及术后12个月,两组的Harris评分比较均无统计学差异(P>0.05);术后6个月,观察组的Harris评分显著低于对照组(P <0.05)。观察组的骨折愈合时间显著短于对照组(P <0.05)。观察组的并发症发生率为10.34%,与对照组的20.69%比较无统计学差异(P>0.05)。结论与3枚空心钉内固定比较,采用4枚空心钉内固定治疗股骨颈骨折患者,可预防骨折不愈合和早期内固定失败,具有更强的抗剪切力作用,值得应用与推广。

Objective To explore the curative effect of internal fixation with different numbers of cannulated screws in the treatment of femoral neck fracture. Methods 58 cases of elderly patients with femoral neck fracture undergoing cannulated screw internal fixation in our hospital from January 2016 to November 2017 were selected and divided into observation group(internal fixation with 4 cannulated screws) and control group(internal fixation with 3 cannulated screws) according to different numbers of cannulated screws, with 29 cases in each group. The treatment effects of the two groups were compared. Results Before operation and 3 months, 9 months and 12 months after operation, no statistical difference was found in the Harris scores between the two groups(P >0.05);6 months after operation, the Harris score of the observation group was significantly lower than that of the control group(P<0.05). The fracture healing time of the observation group was significantly shorter than that of the control group(P<0.05). The incidence of complications of the observation group was10.34%, which had no statistical difference with 20.69% of the control group(P<0.05). Conclusions Compared with internal fixation with 3 cannulated screws, internal fixation with 4 cannulated screws in the treatment of patients with femoral neck fracture can prevent the fracture nonunion and early failure of internal fixation, with stronger anti-shear effect, which is worthy of application and promotion.
