降钙素原、C-反应蛋白和白细胞介素-6在评估糖尿病足感染病情严重程度及预后的价值

The value of PCT, CRP and IL-6 in assessing the severity and prognosis of diabetic foot infection

目的探讨降钙素原(PCT)、C-反应蛋白(CRP)和白介素-6(IL-6)在评估糖尿病足感染病情严重程度和预后中的价值。方法选择本院内分泌科病房2015年1月-2018年3月收治的糖尿病足感染患者132例作为疾病组,分为轻度感染(52例)、中度感染(55例)和重度感染(25例)3组,比较血浆PCT、CRP和IL-6水平的差异和病情严重程度,相关性分析及预后不良的独立危险因素。结果随着DF感染程度的加重,IL-6、PCT和CRP逐渐升高,中度感染组和重度感染组的IL-6、PCT和CRP明显高于轻度感染组,差异均有统计学意义(P<0.01)。DF患者血清IL-6、PCT和CRP与感染分级呈正相关(r值分别为0.58、0.67、0.51,P<0.01)。单因素分析和进一步Logistic回归分析显示糖尿病足病程和PCT是糖尿病足感染预后不良的独立危险因素。结论PCT可以反应糖尿病足感染的疾病严重程度,且可以用来评估患者的预后情况。

Objective To investigate the value of procalcitonin ( PCT),C-reactive protein ( CRP) and interleukin-6 ( IL-6) in assessing the severity and prognosis of diabetic foot infection. Methods A total of 132 patients with diabetic foot infections from January 2015 to March 2018 in our hospital were divided into mild infection group ( 52 cases),moderate infection group ( 55 cases) and severe infection group ( 25 cases). The differences in plasma PCT,CRP,and IL-6 levels and the severity of the disease were compared,and correlation and the independent risk factors for poor prognosis were analyzed. Results With the aggravation of DF infection,IL-6,PCT and CRP increased gradually. IL-6,PCT and CRP in moderate infection group and severe infection group were significantly higher than those in mild infection group,with the differences statistically significant ( P < 0. 01). Serum IL-6,PCT and CRP were positively correlated with infection grade in DF patients ( r = 0. 58,r = 0. 67,r = 0. 51,P < 0. 01). Univariate analysis and further logistic regression analysis showed that diabetic foot disease and PCT were independent risk factors for poor prognosis of diabetic foot infection. Conclusion PCT can reflect the severity of disease in diabetic foot infection and can be used to assess the prognosis of patients.
