后路椎间融合联合内固定治疗胸腰椎骨折脱位的临床疗效观察

Clinical Efficacy of Posterior Lumbar Interbody FusionCombined with Internal Fixationin Treatment of Thoracolumbar Spinal Fracture-Dislocation

目的观察后路椎间融合联合内固定治疗胸腰椎骨折脱位的临床疗效。方法将2016年2月至2017年1月我院治疗的108例胸腰椎骨折脱位患者作为研究对象,将其依照随机数表法分为观察组(54例)和对照组(54例)。对照组实施后路固定术治疗,观察组加用后路椎间融合术治疗。观察两组临床疗效、神经功能恢复情况、治疗前后椎体前后缘高度比值变化情况与并发症发生率等。结果与对照组相比,观察组总有效率相对较高,并发症发生率相对较低,差异有统计学意义(P<0.05);术前两组神经功能恢复等级、椎体前、后缘高度比值对比,差异无统计学意义(P>0.05);术后观察组神经功能恢复等级、椎体前、后缘高度比值优于对照组,差异有统计学意义(P<0.05)。结论后路椎间融合、内固定术联合治疗胸腰椎骨折脱位安全性较高且疗效确切,可减少术后并发症,加快患者神经功能恢复,改善椎体高度比值。

Objective To observe the clinical efficacy of posterior lumbar interbody fusion(PLIF)combined with internal fixation in treatment of thoracolumbar spinal fracture-dislocation.Methods 108 patients with thoracolumbar spinal fracture-dislocation treated in our hospital from February 2016 to January 2017 were selected,and they were divided into observation group(n=54)and control group(n=54)by random number table by random number table.Control group was given posterior fixation,while observation group was treated with PLIF based on control group.The clinical efficacy,recovery of nerve function,ratio of anterior to posterior vertebral body height,complications in two groups were observed.Results Compared with control group,the total effective rate of observation group was higher,while the incidence of complications was lower(P<0.05);Before surgery,there was no statistical difference in the grade of nerve function recovery,ratio of anterior to posterior vertebral body height between the two groups(P>0.05);After surgery,the grade of nerve function recovery,ratio of anterior to posterior vertebral body height in observation group were better than those in control group(P<0.05).Conclusion PLIFcombined with internal fixation in treatment of thoracolumbar spinal fracture-dislocation is safe and effective,it can reduce postoperative complications,accelerate recovery of nerve function,improve ratio of vertebral body height.
