前哨淋巴结活检联合保乳手术治疗老年乳腺癌的效果

Effect of sentinel lymph node biopsy combined with breast conserving surgery in the treatment of elderly breast cancer

目的探讨前哨淋巴结活检联合保乳手术治疗老年乳腺癌的效果。方法选取我院2017年1~12月确诊的76例老年乳腺癌患者作为研究对象,按照随机数字表法分为对照组(36例)与观察组(40例)。对照组采用传统根治术治疗,观察组予以前哨淋巴结活检联合保乳手术治疗,比较两组的治疗效果。结果观察组的手术时间、胸壁引流管拔管时间短于对照组,差异有统计学意义(P<0.05)。观察组的术中出血量、总引流量少于对照组,差异有统计学意义(P<0.05)。观察组的术后美容优良率高于对照组,差异有统计学意义(P<0.05)。观察组的术后生活质量评分、肩关节功能评分高于对照组,局部复发率低于对照组,肩关节外展角度变化、两侧上臂臂围变化范围大于对照组,并发症总发生率低于对照组,差异有统计学意义(P<0.05)。两组患者的总生存率、无病生存率比较,差异无统计学意义(P>0.05)。结论前哨淋巴结活检联合保乳手术治疗老年乳腺癌可促进患者病情早日康复,提高患者的生活质量,是一种安全可靠的老年乳腺癌治疗方法。

Objective To investigate the effect of sentinel lymph node biopsy combined with breast conserving surgery in the treatment of elderly breast cancer. Methods A total of 76 elderly patients with breast cancer diagnosed in our hospital from January 2017 to December 2017 were selected and randomly divided into the control group (36 cases) and the observation group (40 cases). The control group was treated with traditional radical surgery. The observation group was treated with sentinel lymph node biopsy combined with breast-conserving surgery. The therapeutic effects of the two groups were compared. Results The operation time,intraoperative blood loss,and chest tube drainage time of the observation group were shorter than those of the control group,with significant difference (P<0.05). The total drainage volume of the observation group was lower than that of the control group,with significant difference (P<0.05). The postoperative cosmetic excellent rate of the observation group was higher than that of the control group,the postoperative quality of life score and shoulder joint functional score of the observation group were higher than those of the control group,and the local recurrence rate of the observation group was lower than that of the control group,the abduction angle of the shoulder joint and the range of the upper arm circumference of the observation group were larger than those of the control group,the total incidence rate of complication of the observation group was lower than that of the control group,with significant difference (P<0.05). There was no significant difference in the overall survival rate and disease-free survival rate between the two groups (P>0.05). Conclusion Sentinel lymph node biopsy combined with breast-conserving surgery in the treatment of elderly breast cancer can promote the patient′s early recovery,and improve the quality of life of patients,which is a safe and reliable treatment for elderly breast cancer.
