中西医结合治疗克罗恩病合并肛瘘的临床效果

Clinical effect of integrated traditional Chinese and Western medicine on Crohn′s disease complicated with anal fistula

目的探讨中西医结合治疗克罗恩病合并肛瘘的临床效果。方法选取2016年1月~2018年1月我院收治的60例克罗恩病合并肛瘘患者作为研究对象,根据随机数字表法分为观察组和对照组,每组各30例。两组均给予挂线引流术治疗,对照组术后采用高猛酸钾坐浴,观察组术后采用中药方剂进行坐浴及内服参苓白术散。比较两组克罗恩病的简化活动指数(CDAI)、水肿情况、疼痛情况、术后并发症总发生率、创面愈合时间及治疗效果。结果观察组患者治疗后的CDAI评分、水肿评分、疼痛评分显著低于对照组,差异均有统计学意义(P<0.05);观察组患者的流脓和瘙痒发生率低于对照组,术后创面愈合时间短于对照组,差异均有统计学意义(P<0.05);观察组患者的治疗总有效率为96.7%,显著高于对照组的73.3%,差异有统计学意义(P<0.05)。结论中西医结合治疗克罗恩病合并肛瘘的临床效果显著,能促进克罗恩病肛瘘患者窦道愈合,减轻肛周活动度,降低术后疼痛和水肿,值得推广。

Objective To explore the clinical effect of integrated traditional Chinese and Western medicine on Crohn′s disease complicated with anal fistula. Methods A total of 60 patients with Crohn′s disease complicated with anal fistula in our hospital from January 2016 to January 2018 were selected as the research objects,they were divided into observation group and control group,30 cases in each group according to the random number table method. Both groups were treated with thread-drawing and drainage,the control group was treated with potassium permanganate hip bath after operation,while the observation group was treated with traditional Chinese medicine prescription for hip bath and Shenlingbaizhu Powder after operation. The simplified activity index (CDAI),edema,pain,total incidence of complications,wound healing time and therapeutic effect of Crohn′s disease were compared between the two groups. Results After treatment,the CDAI score,edema score and pain score in the observation group were significantly lower than those in the control group,the differences were statistically significant (P<0.05). The incidence of purulence and pruritus in the observation group was lower than that in the control group,and the healing time of the wound was shorter than that in the control group,the differences were statistically significant (P<0.05). The total effective rate was 96.7% in the observation group,which was significantly higher than 73.3% in the control group,the difference was statistically significant (P<0.05). Conclusion The clinical effect of integrated traditional Chinese and Western medicine in the treatment of Crohn′s disease with anal fistula is remarkable. It can promote the sinus healing of Crohn′s disease with anal fistula,alleviate perianal motion,reduce postoperative pain and edema,and is worthy of promotion.
