正畸牵引联合牙冠延长术治疗恒牙外伤断根的效果

Effect of orthodontic traction combined with crown lengthening on patients with permanent teeth traumatic root fracture

目的探究正畸牵引联合牙冠延长术治疗恒牙外伤断根的效果。方法选取我院收治的72例恒牙外伤断根患者作为研究对象,将其随机分为观察组(36例,正畸牵引联合牙冠延长术)和对照组(36例,牙冠延长术),比较两组患者治疗总有效率、治疗前及治疗后2周和3个月牙龈沟出血指数(SBI)、牙周探诊深度(PD)、牙齿松动度(TM)以及治疗满意度。结果治疗后3个月,观察组治疗总有效率显著高于对照组(P<0.05)。两组患者治疗后2周及3个月SBI、PD均显著低于治疗前(P<0.05);观察组治疗后2周及3个月SBI、PD显著低于对照组(P<0.05);观察组治疗后2周及3个月TM水平与对照组比较,差异无统计学意义(P>0.05)。两组患者治疗后6个月满意度评分较治疗后2周有所升高,且观察组显著高于对照组(P<0.05)。结论正畸牵引联合牙冠延长术治疗恒牙外伤断根能改善患者牙周状况,提高患者满意度,疗效显著。

Objective To explore the effect of orthodontic traction combined with crown lengthening on patients with permanent teeth traumatic root fracture. Methods Seventy-two patients with permanent teeth traumatic root fracture in our hospital were selected as study objects, and randomly divided into observation group (36 cases, orthodontic traction combined with crown lengthening) and control group (36 cases, crown lengthening). The total effective rates of treatment, the levels of gingival sulcus bleeding index (SBI), periodontal probing depth (PD), teeth mobility (TM) before treatment and at 2 weeks and 3 months after treatment, treatment satisfaction were compared between the two groups. Results At 3 months after treatment, the total effective rate of treatment in the observation group was significantly higher than that in the control group (P<0.05). At 2 weeks and 3 months after treatment, the SBI and PD in both groups were significantly better than those before treatment (P<0.05), and the SBI and PD in the observation group were significantly lower than those in the control group (P<0.05). There was no significant difference in TM at 2 weeks and 3 months after treatment between the observation group and the control group (P>0.05). The patients' satisfaction scores of the two groups at 6 months after treatment were higher than those at 2 weeks after treatment, and that of the observation group was significantly higher than he control group (P<0.05). Conclusion Orthodontic traction combined with crown lengthening can improve the periodontal condition and satisfaction of patients with permanent teeth traumatic root fracture, with remarkable curative effect.
