摘要 目的探讨Sysmex XE-5000血细胞分析仪对低值血小板检测的准确性。方法以90例血小板计数减低标本为研究对象,分别运用红细胞/血小板检测通道的鞘流电阻抗法(PLT-I)、网织红细胞通道的核酸荧光染色结合流式法(PLT-O)和人工显微镜法检测血小板计数。结果当仪器提示有大血小板存在,且未成熟血小板比率(IPF)≥25%时,PLT-I、PLT-O和人工显微镜法检测血小板计数比较,差异均有统计学意义(P<0.05),但PLT-O法和人工显微镜法比较,差异无统计学意义(P>0.05);当仪器仅提示血小板计数减低、无血小板形态和成熟度异常时,三种方法检测血小板计数比较,差异无统计学意义(P>0.05)。结论在常规工作中,Sysmex XE-5000细胞分析检测仪的两种血小板检测方法均具有较好的准确性和稳定性,当血小板计数减低、直方图报告异常、未成熟血小板比率明显升高时,建议采用PLT-O法进行复检,必要时行人工镜检,确保为临床提供准确、可靠的检测结果。 Objective To evaluate the accuracy of Sysmex XE-5000 hematology analyzer on detecting low-value patelets. Methods Ninety cases of low-value patelets were enrolled as the study objects. Platelets were counted by sheath flow impedance(PLT-I) of erythrocyte/platelet detection channel, nucleic acid fluorescence staining of reticulocyte channel combined with flow cytometry(PLT-O) and artificial microscopy respectively. Results When the instrument indicated the presence of large platelets and the immature platelet ratio(IPF)≥25%, there were significant differences between PLT-I,PLT-O and artificial microscopy(P<0.05), but there was no significant difference between PLT-O and artificial microscopy(P >0.05). When the instrument only indicated the decrease of platelet count, platelet-free morphology and maturity abnormality, there was no significant difference between the three methods(P>0.05). Conclusion In routine work, the two platelet detection methods of Sysmex XE-5000 have good accuracy and stability. When the platelet count decreases, the histogram report is abnormal and the ratio of immature platelet increases significantly. PLT-O method is recommended for re-examination and artificial microscopy when necessary to ensure accurate and reliable results for clinical detection.
