摘要
目的采用分层应变技术评价非梗阻型肥厚型心肌病(NHCM)患者左心室收缩功能,为临床心功能的评价提供定量信息。方法选取西电集团医院收治的42例NHCM患者(NHCM组)与48例健康体检者(对照组),获取各心尖长轴水平超声二维动态图像进行存储,脱机采用EchoPAC 113软件测量各切面心内膜下、中层及心外膜下心肌整体纵向应变峰值(GLS-endo、GLS-mid、GLS-epi)。结果两组的GLS-endo、GLS-mid、GLS-epi均表现为梯度递减(P<0.001);NHCM组左心室GLS-endo、GLS-mid、GLS-epi均低于对照组(P<0.001)。NHCM组GLS-endo、GLS-mid、GLS-epi绝对值与最大室壁厚度呈线性负相关(P<0.001)。结论分层应变技术是评价NHCM患者左心室纵向收缩功能的一种新技术。
Objective The left ventricular systolic function in patients with non-obstructive hypertrophic cardiomyopathy (NHCM) was evaluated by stratified strain technique, which provided quantitative information for clinical evaluation of cardiac function. Methods A total of 42 NHCM patients (NHCM group) admitted in Xi'an XD group hospital and 48 healthy controls (control group) were selected. Two-dimensional dynamic echocardiographic images of each apex were acquired. The longitudinal strain peak values of the subendocardial and middle layer and epicardial inferior of each section (GLS-endo, GLS-mid, GLS-epi) were measured offline using EchoPAC 113 software. Results GLS-endo, GLS-mid and GLS-epi in both groups showed gradient decrease (P<0.001). The left ventricular GLS-endo, GLS-mid and GLS-epi in the NHCM group were lower than those in the control group (P<0.001). The absolute values of GLS-endo, GLS-mid and GLSepi in the NHCM group were negatively correlated with the maximum wall thickness (P<0.001). Conclusion Stratified strain technique is a new technique for evaluating left ventricular longitudinal systolic function in NHCM patients.
