齐刺结合扬刺治疗腰椎间盘突出症临床观察

Clinical Observation of Triple Needling Combined with Central-Square Needling Treating LDH

目的:观察齐刺结合扬刺治疗腰椎间盘突出症的临床疗效。方法:将符合纳入标准的60例腰椎间盘突出症患者随机分为齐刺结合扬刺组和常规针刺组两组,每组30例。齐刺结合扬刺组以下极俞、腰阳关、十七椎、第1骶椎棘突下、气海俞、大肠俞、关元俞、小肠俞为主施以齐刺结合扬刺针法;常规针刺组以双侧气海俞、大肠俞、关元俞、小肠俞为主,施以常规针刺。两组均留针30min,每日1次,10次为1个疗程,疗程间休息1天。2个疗程后,观察治疗前后JOA评分、VAS评分的变化,比较各组疗效。结果:齐刺结合扬刺组有效率为100.00%(30/30),优于常规针刺组的83.33%(25/30),差异具有统计学意义(χ^2=9.333,P<0.05);两组治疗后JOA评分均较治疗前明显提高(均P<0.05),且齐刺结合扬刺组评分高于常规针刺组(P<0.05);两组治疗后VAS评分均降低,且两组比较差异有统计学意义(均P<0.05)。结论:齐刺配合扬刺是一种治疗腰椎间盘突出症的有效方法,临床疗效肯定。

Objective: To observe the clinical effect of tirple needling combined with central-square needling in the treatment of lumbar disc herniation(LDH). Methods: 60 LDH patients were randomly divided into the the combination group( n =30) and the common acupuncture group( n =30). The combination group was treated with triple needling combined with central-square needling, the points chosen were EX-B5, GV3, EX-B8, BL24, BL25, BL26, BL27, and subspinous process of the first sacral vertebra. Whereas, BL24, BL25, BL26, and BL27 were selected in the common acupuncture group. The treatment was once a day, ten days were taken as one course of treatment, the whole treatment duration was two courses with one day interval of each course. The changes of JOA scores and VAS scores were observed before and after the treatment in the two groups to evaluate the efficacy. Results: The total effective rate was 100%(30/30) in the combination group, which was obviously higher than 83.33%(25/30) in the common acupuncture group(χ^2=9.333, P <0.05). JOA scores were significantly increased after the treatment compared to those before the treatment in the two groups( P <0.05), and they were much higher in the combination group than those in the common acupuncture group( P <0.05). VAS scores were decreased after the treatment in the two groups( P <0.05). Conclusion: Triple needling combined with central-square needling is an effective therapy for LDH with its positive efficacy.
