持续性护理在离院糖尿病患者中的应用

Application of Continuous Nursing in Patients with Diabetes Mellitus Discharged from Hospital

目的探讨持续性护理在离院糖尿病患者中的应用效果。方法选择2017年4月—2018年4月于我院住院治疗的2型糖尿病患者96例,将其以随机数表法进行分组。其中对照组48例采取院内常规护理与出院指导,研究组48例患者在此基础上采用持续性护理。评价两组离院时与离院6个月后血糖指标(空腹血糖、餐后2h血糖与糖化血红蛋白)的变化,以及离院6个月后自我管理能力的变化。结果离院6个月时,研究组空腹血糖、餐后2h血糖与糖化血红蛋白低于对照组,差异有统计学意义(P<0.05),遵医用药、合理运动、自我监测与合理饮食评分均高于对照组,差异有统计学意义(P<0.05)。结论持续性护理在离院糖尿病患者中具有显著的应用效果,值得推广。

Objective To explore the effect of continuous nursing on diabetic patients discharged from hospital.Methods 96 patients with type 2 diabetes mellitus in our hospital from April 2017 to April 2018 were divided into control group and research group,48 cases in each group.The control group was given routine nursing,while the research group was given continuous nursing on the basis of routine nursing.The changes in blood glucose levels(fasting blood glucose,postprandial 2 h blood glucose and glycosylated hemoglobin)when leaving hospital and 6 months after leaving the hospital were evaluated,as well as changes in selfmanagement ability after 6 months of hospitalization.Results At 6 months after discharge,fasting blood glucose,2 h postprandial blood glucose and glycosylated hemoglobin in the study group were lower than those in the control group,the difference between the two groups was statistically significant(P<0.05),and the total scores of compliance with medicines,reasonable exercise,self-monitoring,and reasonable diet in the study group were higher than those in the control group,the difference between the two groups was statistically significant(P<0.05).Conclusion Continuous nursing has significant application effect in patients with diabetes mellitus who are discharged from hospital,and it is worthy of popularizing.
