三胶膏联合体外冲击波治疗股骨头坏死临床观察

Clinical observation of Sanjiao paste combined with extracorpor eal shock wave in the treatment of femoral head necrosis

目的:观察体外冲击波联合三胶膏治疗股骨头坏死的临床疗效。方法:将符合标准的60例股骨头坏死患者(ARCO分期0-Ⅱ期)随机分为2组,其中,对照组30例,给予体外冲击波辅助功能锻炼;观察组30例,在对照组治疗基础上配合口服三胶膏治疗。以上2组同时治疗2个月。治疗前,对所有患者的髋关节功能评分(Harris评分)、视觉模拟评分法(VAS)以及血清C-反应蛋白(C-reactive protein,CRP)进行收集整理,按照上述治疗方法治疗2个月后对上述指标再次进行收集整理。结果:对照组总有效率为63.33%,其中,2例治愈,显效8例,有效9例,无效11例;观察组总有效率为86.67%,其中,16例治愈,显效6例,好转4例,无效4例。2组患者治疗后的髋关节功能评分(Harris评分)、视觉模拟评分法(VAS)以及血清CRP均有明显改善。结论:体外冲击波联合三胶膏治疗股骨头坏死具有较好的临床效果,改善股骨头坏死患者相关临床指标以及临床症状,提高患者的生活质量,增加保守治疗的成功率,起到1+1>2的临床效果。

Objective: To observe the clinical effect of extracorporeal shock wave combined with Sanjiao paste in the treatment of femoral head necrosis. Methods: 60 patients with osteonecrosis of femoral head(ARCO stage 0-Ⅱ)were randomly divided into two groups: the control group(n=30) received extracorporeal shock wave assisted exercise,the observation group(n=30) was treated with oral Sanjiao ointment on the basis of the treatment of the control group.Both groups were treated for 2 months at the same time.Before treatment, the hip function score(Harris), visual analogue score(VAS) and serum C-reactive protein(C-reactive protein, CRP) were collected and sorted. After 2 months of treatment, the above indexes were collected and sorted again. Results: The total effective rate of the control group was 63.33%, of which 2 cases were cured, 8 cases were markedly effective, 9 cases were effective, and 11 cases were ineffective. The total effective rate of the observation group was 86.67%, of which 16 cases were cured, 6 cases were markedly effective, and 4 cases were improved, Invalid 4 cases.After treatment, the hip function score(Harris), visual analogue score(VAS) and serum CRP were significantly improved in both groups. Conclusion: The treatment of femoral head necrosis with extracorporeal shock wave combined with Sanjiao paste has a good clinical effect. The clinical indexes and clinical symptoms of patients with femoral head necrosis are improved, the quality of life of patients is im-proved, and the success rate of conservative treatment is increased clinical effect.
