PVP手术对骨质疏松性椎体压缩骨折患者肺功能的影响 被引量:7

Effect of percutaneous vertebroplasty on pulmonary function in patients with osteoporotic vertebral compression fractures

摘要 目的探讨经皮椎体成形术(PVP)对骨质疏松性椎体压缩骨折患者肺功能的影响。方法回顾性分析自2016-09—2017-09采用PVP手术治疗的70例骨质疏松性椎体压缩骨折,比较术前、术后3 d、术后3个月疼痛VAS评分、伤椎Cobb角、用力肺活量(FVC)、第1秒用力呼气量(FEV1)、1秒用力呼气容积(FEV1/FVC)、每分钟最大通气量(MVV)。结果 70例均获得3个月随访。术后3 d疼痛VAS评分、伤椎Cobb角较术前改善,且术后3个月较术后3 d改善,差异有统计学意义(P <0.05)。术后3 d的FVC与术前比较差异无统计学意义(P>0.05);术后3个月FVC较术后3 d增加,差异有统计学意义(P <0.05)。术后3 d的FEV1较术前增加,差异有统计学意义(P <0.05);但术后3个月与术后3 d的FEV1差异无统计学意义(P>0.05)。术后3 d的FEV1/FVC较术前降低,且术后3个月低于术后3 d;术后3 d的MVV较术前增加,且术后3个月高于术后3 d,差异有统计学意义(P <0.05)。结论 PVP手术能够有效减轻骨质疏松性胸腰椎压缩骨折患者疼痛,改善脊柱后凸畸形,从而提高患者的日常活动能力,进一步使患者肺功能得到改善。 Objective To investigate the effect of percutaneous vertebroplasty(PVP)on pulmonary function in patients with osteoporotic vertebral compression fractures.Methods Retrospective analysis of 70 cases of osteoporotic vertebral compression fractures treated with PVP surgery from September 2016 to September 2017 was conducted.The pain VAS score,injured Cobb angle,forced vital capacity(FVC),forced expiratory volume in 1 second(FEV1),forced expiratory volume in 1 second(FEV1/FVC),maximum ventilation per minute(MVV)before operation and 3 d,3 months after operation were compared.Results All 70 patients were followed up for 3 months.The pain VAS score and the Cobb angle of the injured vertebra were improved at 3 days after operation,and improved at 3 months after operation,and the difference was statistically significant(P<0.05).There was no significant difference in FVC between the 3 rd day after operation and the preoperative period(P>0.05).The FVC increased 3 months after operation,and the difference was statistically significant(P<0.05).The FEV1 increased 3 days after operation,and the difference was statistically significant(P<0.05).However,there was no significant difference in FEV1 between 3 months and 3 days after operation(P>0.05).The FEV1/FVC decreased 3 days after operation,and the FEV1/FVC in 3 months after operation was lower than 3 days after operation.The MVV increased 3 days after operation,and the MVV in 3 months after operation was higher than 3 days after operation.The difference was statistically significant(P<0.05).Conclusion PVP surgery can effectively alleviate the pain of patients with osteoporotic thoracolumbar vertebral compression fractures,improve the kyphosis deformity,thereby improving the daily activities of patients,and further improve the lung function of patients.
