基于895份住院病例数据挖掘的中医药治疗强直性脊柱炎用药规律研究

Study on the Drug Use Rule of Traditional Chinese Medicine in Treating Ankylosing Spondylitis Based on 895 Hospitalited Cases Data Mining

目的:利用数据挖掘技术和关联规则分析研究安徽中医药大学第一附属医院(简称本院)风湿科治疗强直性脊柱炎(AS)的治疗规律、处方用药规律。方法:基于数据挖掘技术,对本院风湿免疫科2013年6月—2017年6月住院AS患者的病例资料进行回顾性研究。使用SPSS21.0及SPSS Clementine 11.1对患者的中药处方及其性味归经进行频数统计、关联规则分析。结果:共有895份病历、335味中药及4 125张处方被纳入研究,主要分析结果:(1)药物性味归经使用频数统计结果:温性(4 074次)、平性(4 046次)及寒性(3 193次),甘味(4 088次)、辛味(4 075次)及苦味(4 057次),归肝、脾、胃经中药使用最为频繁。(2)常用药物分为健脾祛湿药、祛风湿药、活血化瘀药、补虚药、清热解毒药五类。(3)核心处方为陈皮、茯苓、薏苡仁、泽泻、山药、威灵仙、豨莶草、狗脊、丹参、桃仁、红花、蒲公英、白花蛇舌草、杜仲、甘草。(4)药物关联规则分析结果:红花与鸡血藤、蒲公英与白花蛇舌草、茯苓与薏苡仁为关联度较高的3个药对。结论:本院治疗AS主以健脾化湿,配伍祛风除湿、活血化瘀、补虚、清热解毒之法,寒温并用,辛甘合用,肝脾同治,扶正祛邪,标本兼治。

Objective:Data mining technique and association rule analysis were used to study the treatment rule and prescription of(AS)in the Department of Rheumatology of the first affiliated Hospital of Anhui University of Chinese Medicine(Anhui Provincial Hospital of Chinese Medicine)in the treatment of ankylosing spondylitis.Methods:Based on data mining technology,a retrospective study was carried out on the data of patients in Rheumatological Immunology Department of the first affiliated Hospital of Anhui University of Chinese Medicine.SPSS21.0 and SPSS Clementine 11.1 were used to analyze the frequency and association rules of TCM prescriptions.Results:A total of 895 medical records,335 traditional Chinese medicines and 4125 prescriptions were included in the study.The main results were as follows:(1)Statistical results of the frequency of medicinal flavor meridian attribution:warmth(4074 times),flatness(4046 times)and cold(3193 times),sweetness(4088 times),spicy(4075 times)and bitterness(4057 times),liver,spleen,stomach meridian Chinese medicines were used most frequently.(2)Rheumatology Department of our hospital.There are five kinds of drugs commonly used to treat AS:invigorating spleen and removing dampness,eliminating rheumatism,activating blood circulation and removing stasis,tonifying deficiency,clearing away heat and detoxifying.Coix seed,Alisma orientalis,Chinese yam,Clematis,Herba Siegesbeckiae,Canine spine,Salvia miltiorrhiza,Peach kernel,Safflower,Dandelion,Hedyotis diffusa,Eucommia ulmoides ulmoides,Glycyrrhiza uralensis.(4)The results of drug association rules analysis showed that Safflower and Caulis serrata,Dandelion and Hedyotis diffusa,Poria cocos and Coix were three drug pairs with high correlation.Conclusions:under the guidance of Professor Liu Jian,rheumatism department of our hospital treated AS mainly by invigorating spleen and removing dampness,combining with dispelling wind and removing dampness,activating blood circulation and removing blood stasis,tonifying deficiency and clearing away heat and detoxification.Combined with cold and temperature,combined with Xinjan,liver and spleen treatment,nourishing and dispelling evil,both symptoms and symptoms.
