血清miR-206、CA724和PGⅠ联合检测在胃癌诊断和预后评估中的价值

Value of combined detection of serum miR-206,CA724 and pepsinogenⅠin diagnosis and prognosis evaluation of gastric cancer

目的探讨血清微小RNA-206(miR-206)、糖类抗原724(CA724)和胃蛋白酶原Ⅰ(PGⅠ)联合检测在胃癌诊断和预后评估中的价值。方法收集2014年8月至2015年6月该院确诊的200例胃癌患者纳入胃癌组,收集同期该院健康体检者200例纳入健康对照组,检测两组miR-206、CA724和PGⅠ水平。结果相对于健康人群,胃癌患者血清miR-206和PGⅠ水平降低,而CA724水平增高,差异均有统计学意义(P<0.05)。血清PGⅠ、miR-206和CA724鉴别诊断胃癌与健康人群的敏感度/特异度分别为78.5%/86.9%、78.7%/90.7%和70.2%/59.3%;对应的临界值分别为:43.52μg/L、8.84和5.86U/mL。3项指标联合检测诊断胃癌的敏感度为92.3%,特异度为83.4%。相对于血清低miR-206、低PGⅠ与高CA724水平的患者,高miR-206、高PGⅠ与低CA724水平的患者的累积生存率提高,差异均有统计学意义(P<0.05)。低miR-206、低PGⅠ与高CA724水平是胃癌发病的独立危险因素(P<0.05)。结论血清miR-206、CA724和PGⅠ联合检测对胃癌具有较高的诊断及预后评估价值。

Objective To explore the value of combined detection of serum microRNA-206(miR-206),carbohydrate antigen 724(CA724)and pepsinogenⅠ(PGⅠ)in the diagnosis and prognosis evaluation of gastric cancer.Methods Two hundreds patients with gastric cancer diagnosed in this hospital from August 2014 to June 2015 were collected as the gastric cancer group and contemporaneous 200 people undergoing the healthy physical examination in this hospital were colleted as the healthy control group.Then the levels of miR-206,CA724 and PGⅠwere detected.Results Compared with healthy people,the levels of serum miR-206 and PGⅠin gastric cancer patients were decreased,while the CA724 level was increased,the difference was statistically significant(P<0.05).The sensitivity/specificity of serum PGⅠ,miR-206 and CA724 in the differential diagnosis of gastric cancer and healthy people were 78.5%/86.9%,78.7%/90.7%and 70.2%/59.3%respectively.The corresponding critical values were 43.52μg/L,8.84 and 5.86 U/mL.The sensitivity of 3-index combined detection in the diagnosis of gastric cancer was 92.3%and the specificity was 83.4%.Compared with the patients with serum low miR-206,low PGⅠand high CA724 levels,the cumulative survival rate of the patients with high miR-206,high PGⅠand low CA724 levels was statistically increased,and the difference was statistically significant(P<0.05).Low miR-206,low PGⅠand high CA724 levels were the independent risk factors for gastric cancer occurrence(P<0.05).Conclusion The combined detection of serum miR-206,CA724 and PGⅠhas high diagnostic and prognostic value for gastric cancer.
