米索前列醇与缩宫素联合用药对预防瘢痕子宫阴道分娩产后出血的效果分析

The Effect of the Combination of Misoprostol and Oxytocin on the Prevention of Postpartum Hemorrhage in the Vaginal Delivery of the Uterine Vagina

目的:观察瘢痕子宫阴道分娩患者应用米索前列醇与缩宫素联合用药治疗对产后出血的预防价值。方法:回顾性分析2018年1月至2018年12月我院收治的41例瘢痕子宫经阴道分娩产妇临床资料。依据用药方法不同分为观察组(米索前列醇+缩宫素联合用药,n=21例)与对照组(单用缩宫素,n=20例)。在胎肩娩出后,对照组单用缩宫素治疗,观察组使用米索前列醇与缩宫素联合用药治疗。比较两组产后出血情况、产程及不良反应发生情况。结果:两组产后出血发生率相比,差异无统计学意义(P>0.05);观察组产后出血量低于对照组,差异有统计学意义(P<0.05);观察组第三产程短于对照组,差异有统计学意义(P<0.05);两组不良反应发生率相比,差异无统计学意义(P>0.05)。结论:针对瘢痕子宫产妇,需严格把握阴道分娩指征,并在胎肩娩出后应用缩宫素与米索前列醇联合用药治疗可减少产后出血的发生,降低产后出血量,缩短第三产程,且安全性较高。

Objective:To observe the value of the combination of misoprostol and oxytocin in the treatment of postpartum hemorrhage.Methods:The clinical data of 41 cases of uterine transvaginal delivery in our hospital from January 2018 to December 2018 were analyzed retrospectively.According to the method of administration,it was divided into observation group(Misoprostol+oxytocin group),n=21 cases)and control group(oxytocin alone,n=20 cases).After delivery,the control group was treated with oxytocin alone,and the observation group was treated with misoprostol and oxytocin.Postpartum hemorrhage stages of labor the adverse reaction in the two groups was compared..Results:There was no significant difference in the incidence of postpartum hemorrhage between the two groups(P>0.05);the postpartum hemorrhage of the observation group was lower than that of the control group(P<0.05);and the third stage of labor in the observation group was shorter than that of the control group(P<0.05).There was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion:It is necessary to strictly grasp the indication of vaginal delivery and to use the combination of oxytocin and misoprostol to reduce the occurrence of postpartum hemorrhage and to reduce the amount of postpartum hemorrhage.The third stage of labor is shortened and the safety is high.
