基于关联规则的杨晋翔教授治疗反流性食管炎的用药规律

Medication law in professor Yang Jinxiang’s treatment of reflux esophagitis based on the association rule

目的总结杨晋翔教授治疗反流性食管炎的用药规律。方法采集杨晋翔教授治疗反流性食管炎患者有效的156张首诊中药处方。录入到“中医传承辅助平台”软件中,运用软件中频数分析及关联规则的方法进行数据挖掘。结果156张处方中共有173味中药,所用药物多归肝、胃、脾经。其中,寒性药和温性药使用频次较多,苦、辛、甘味药使用频次较多。关联规则分析所得药对及药组中,置信度>98%的规则有37条,置信度为1的有11条,衍化得到1个核心处方为:青蒿、徐长卿、吴茱萸、黄连、车前子、茵陈、煅瓦楞子、浙贝母、炒白术、海螵蛸、代赭石、旋覆花。结论药对及药组分析结果体现了左金丸、乌贝散及旋覆代赭汤的组方思想;四气五味归经及核心处方体现了杨晋翔教授治疗反流性食管炎时从肝脾胃论治,重视化痰祛湿、清降胃气,同时注重辨病与辨证相结合。

Objective To summarize the medication law in professor Yang Jinxiang’s treatment of reflux esophagitis based on the association rule. Methods A total of 156 herbal formulas were collected in the patients with reflux esophagitis at the first visit treated by professor Yang Jinxiang. The data were input in the software of Traditional Chinese medicine inheritance support platform system. Using frequency analysis and association rules,the data were mined. Results In 156 herbal formulas,there were 173 kinds of Chinese herbs and all of them act on liver,stomach and spleen meridians.Of these Chinese herbs,the herbs of cold and warm nature,and bitter,pungent and sweet in flavors were used in a quite high frequency. Among the herb pairs and compositions,with the association rules,the confidence coefficient was > 98% in 37 items and was 1 in 11 items. One core formula was derived,composed of Artemisia aplacea,cyanchum paniculatum,fructus evodiae,coptis chinensis,semen plantaginis,capillary Artemisia,calcined concha arcae,Thunberg fritillary bulb,fried rhizome atractylodis macrocephalae,ossa sepiae,ruddle and inula flower. Conclusion The results of the analysis on paired herbs and herbal compositions embody the ideas of zuojin wan,wubei san and xuanfu daizhe tang. By analyzing the nature,flavor and meridian relationships of the herbs and core formula,it is reflected that professor Yang Jinxiang focuses on resolving phlegm,removing damp and reducing stomach qi on the base of the theory of liver,spleen and stomach in treatment of reflux esophagitis,as well as the integration of disease analysis and TCM pattern identification.
