摘要
目的 :研究在单肺通气下进行手术对接受吸入麻醉患儿肺部摄取麻醉药的影响。方法 :选取2014年2月至2018年7月在广西壮族自治区妇幼保健院进行单肺通气下手术的18例患儿作为观察组,选取同期在该院进行双肺通气下手术的18例患儿作为对照组。为两组患儿均应用七氟醚进行吸入麻醉。然后对比两组患儿的脑电双频指数(bispectralindex,BIS)和呼出麻醉气体的浓度(Et)/吸入麻醉气体的浓度(Fi)。结果 :在开始吸入七氟醚后5 min、10 min、20 min、30 min、40 min、50 min、60 min、70 min、80 min、90 min及100 min,观察组患儿的BIS均高于对照组患儿,P <0.05。在开始吸入七氟醚后10min、20min、30min、40min、50min、60min、70min、80min、90min、100min、110min及120min,观察组患儿的Et/Fi均高于对照组患儿,P <0.05。结论 :在单肺通气下进行手术可影响接受吸入麻醉的患儿肺部对麻醉药的摄取量。
objective: to study the effect of single lung ventilation on pulmonary uptake of anesthetics in children undergoing inhalation anesthesia. Methods: from February 2014 to July 2018, 18 children undergoing single-lung ventilation surgery in the maternal and child health hospital of guangxi zhuang autonomous region were selected as the observation group, and 18 children undergoing double-lung ventilation surgery in the hospital during the same period were selected as the control group.Sevoflurane was used for inhalation anesthesia in both groups. Then compared two groups of children with brain electrical double frequency index(bispectral index, BIS)and exhaled anesthetic gas concentration(Et)/inhalation anesthesia gas concentration(Fi). Results: after inhaling sevoflurane for 5 min, 10 min, 20 min, 30 min, 40 min,50 min, 60 min, 70 min, 80 min, 90 min and 100 min, BIS in the observation group was higher than that in the control group, P < 0.05. At 10 min, 20 min, 30 min, 40 min, 50 min, 60 min, 70 min, 80 min, 90 min, 100 min, 110 min and 120 min after the start of sevoflurane inhalation, the Et/Fi values of the children in the observation group were all higher than those in the control group(P < 0.05). Conclusion: single lung ventilation may affect the lung uptake of anesthetics in children undergoing inhaled anesthesia.
