慢性阻塞性肺疾病急性发作期患者血清缺氧诱导因子-1α与肺功能、血气分析的相关性研究 被引量:5

摘要 目的探讨慢性阻塞性肺疾病(COPD)急性发作期患者血清缺氧诱导因子-1α(HIF-1α)与肺功能、血气分析的相关性。方法选择2015年10月至2017年10月在我院进行治疗的急性发作期COPD患者100例作为观察组,另外选择同期在我科进行治疗的肺炎、支气管炎患者50例作为对照组。测定患者肺功能、血气分析,血清HIF-1α。评价其相关性。结果观察组二氧化碳分压(PCO2)明显高于对照组(P<0.01);观察组第1秒用力呼气容积实测值占预计值的百分比(FEV1)、第1秒用力呼气容积占用力肺活量百分比(FEV1/FVC)、氧分压(PO2)均明显低于对照组(P<0.01)。经Pearson相关分析显示,血清HIF-1α与FEV1、FEV1/FVC、PO2均呈负相关(r=-0.601、-0.416、-0.339,P<0.01)。将血清HIF-1α作为应变量,性别、年龄、升高、体质量、PO2、PCO2、FEV1、FEV1/FVC作为自变量进行分析,HIF-1α与性别、年龄、升高、体质量均无线性回归关系;HIF-1α与PO2、FEV1、FEV1/FVC有线性回归关系。结论COPD患者血清HIF-1α与肺功能、血气分析有显著的相关性,有助于临床对COPD病情进行分析。 Objective To study the correlation between serum hypoxia inducible factor-1 alpha(HIF-1α)and pulmonary function and blood gas analysis in patients with acute stage of chronic obstructive pulmonary disease(COPD).Methods 100 patients with acute exacerbation of COPD who were treated in our hospital from October 2015 to October-2017 were selected as the observation group,and 50 patients with pneumonia and bronchitis treated in our department during the same period were selected as the control group.The patients'pulmonary function,blood gas analysis and serum HIF-1αwere measured.Evaluation of its relevance.Results The partial pressure of carbon dioxide(PCO2)in the observation group was significantly higher than that of the control group(P<0.01).The percentage of forced expiratory volume in the first second of the observation group(FEV1),the forced expiratory volume in the first second,the percentage of forced vital capacity(FEV1/FVC)and the oxygen partial pressure(PO2)in the observation group were significantly lower than those in the control group(P<0.01).Pearson correlation analysis showed that serum HIF-1 alpha was negatively correlated with FEV1,FEV1/FVC and PO2(r=-0.601,-0.416,-0.339,P<0.01).The serum levels of HIF-1αas the dependent variables,gender,age,and increased body mass,PO2,PCO2,FEV1,FEV1/FVC as independent variables,gender,age,and HIF-1 alpha,increased body mass were not linear regression;HIF-1,FEV1,FEV1/FVC alpha and PO2 had linear regression relation.Conclusions There is a significant correlation between the serum HIF-1 alpha in the patients with COPD and the analysis of pulmonary function and blood gas analysis,which is helpful to the analysis of the clinical condition of COPD.
