大承气汤治疗胸腰椎骨折术后腹胀便秘临床观察

Clinical Observation on Dachengqi Decoction on Abdominal Distension Constipation after Thoracolumbar Fracture Surgery

目的探讨大承气汤加味对胸腰椎骨折术后腹胀便秘的治疗效果。方法选取收治的62例行胸腰骨折手术后腹胀便秘患者为研究对象,随机分为研究组和对照组,各31例。对照组采取西医常规治疗,研究组在对照组的基础上采用大承气汤治疗,2组均连续治疗7 d。记录2组肛门排气时间、排便次数及首次排便时间,比较2组患者治疗前后疼痛情况,统计2组治疗后临床有效率。结果研究组肛门排气时间及首次排便时间较对照组显著缩短,排便次数较对照组显著增多(P <0. 01)。自治疗前至治疗7 d后,2组VAS评分逐渐降低(P <0. 01),且在治疗后各个时间点研究组显著低于对照组(P <0. 01)。研究组总有效率显著高于对照组(P <0. 01)。结论大承气汤加味可有效缓解胸腰椎骨折术后患者腹胀便秘的临床症状,缩短排气排便时间,且效果显著优于西医常规治疗。

Objective To investigate the clinical effect of modified Dachengqi decoction on abdominal distension constipation after thoracolumbar fracture surgery.Methods 62 patients with abdominal distension constipation after thoracolumbar fracture surgery admitted to our hospital were selected as the study objects,and they were randomly divided into study group and control group,with 31 cases in each group.The control group was treated with conventional western medicine,and the study group was treated with Dachengqi decoction on the basis of the control group.2 groups were treated continuously for 7 d.The anal exhaust time,defecation frequency and first bowel movement of the two groups were recorded,the pain conditions of 2 groups before and after treatment were compared,and the clinical efficiency of 2 groups after treatment was calculated.Results The time of anal exhaust and first bowel movement in the study group was significantly shorter than those in the control group,and the frequency of defecation was significantly higher than that in the control group(P<0.01).The VAS score of 2 groups decreased gradually from before treatment to 7 d after treatment(P<0.01),and the study group was significantly lower than the control group at each time point after treatment(P<0.01).The total effective rate of the study group was significantly higher than that of the control group(P<0.01).Conclusion Modified Dachengqi decoction can effectively relieve the clinical symptoms of abdominal distension constipation after thoracolumbar fracture surgery,shorten the exhaust and defecate time,and the effect is significantly better than the conventional treatment of western medicine.
