中药外敷联合TDP照射和下肢牵引三位一体疗法治疗早中期股骨头坏死

External Application of Traditional Chinese Medicine Combined with TDP Irradiation and Lower Limb Traction Trinity Therapy for Early and Middle Femoral Head Necrosis

目的临床研究分析中药外敷联合TDP和牵引治疗早中期股骨头坏死的疗效。方法方便选取2014年6月—2018年4月于秦皇岛市中医医院骨伤科住院的早中期股骨头坏死患者60例,单盲随机分成中药外敷联合TDP照射和下肢牵引治疗组(观察组)30例,和行髓芯减压自体骨植骨术组(对照组)30例,分别评价患者治疗前及治疗后2周、6周、12周和6个月的疼痛情况(VAS评分),以及治疗前和治疗后3个月、6个月髋关节功能情况(Harris评分)。结果治疗2周时VAS评分观察组(3.40±0.74)分,对照组(3.81±0.68)分,两组比较差异有统计学意义(t=2.24,P=0.029)。治疗6周及6个月时,两组比较,差异无统计学意义(t=-0.47,-0.52,P=0.64,0.60)。治疗后3个月和6个月时两组的Harris评分比较,差异无统计学意义(t=0.46,0.22,P=0.65,0.83)。结论中药外敷联合TDP照射和下肢牵引三位一体疗法治疗早中期股骨头坏死具有明显疗效。

Objective To analyze the efficacy of external application of traditional Chinese medicine combined with TDP and traction in the treatment of early and middle femoral head necrosis. Methods Convenient select sixty patients with early-stage femoral head necrosis who were hospitalized in the Department of Orthopaedics, Qinhuangdao Traditional Chinese Medicine Hospital from June 2014 to April 2018 were randomly divided into Chinese medicine external application combined with TDP irradiation and lower limb traction treatment group (observation group) and the core-core decompression autogenous bone grafting group (control group), 30 patients were evaluated for pain (VAS score) before treatment and 2 weeks, 6 weeks, 12 weeks and 6 months after treatment, and before treatment and hip function (Harris score) before and 3 months and 6 months after treatment. Results At 2 weeks, the VAS score was observed in the observation group (3.40±0.74)points, and the control group (3.81±0.68)points, after comparison, which had significant difference(t=2.24,P=0.029). At 6 weeks and 6 months of treatment, respectively, with no significant difference(t=-0.47,-0.52, P=0.64, 0.60). At 3 months and 6 months after treatment. There was no significant difference(t=0.46,0.22,P=0.65,0.85). Conclusion External application of traditional Chinese medicine combined with TDP irradiation and lower limb traction triad therapy has obvious curative effect on early and middle femoral head necrosis.
