摘要 目的探讨分析对心肌致密化不全患者的超声影像特点,研究使用彩色多普勒超声心动图诊断和鉴别心肌致密化不全患者的应用。方法方便选取该院2015年11月—2017年11月收治的38例心肌致密化不全患者,对其使用彩色多普勒超声心动图进行扫查,主要是针对心脏扩大以及临床心功能不全患者,以此方式得知患者的心脏结构,血流信号以及形态,尤其是观察患者心尖部以及侧壁的中间段和心尖段的心内膜以及心肌。结果所有患者经过彩色多普勒超声心动图扫查之后,有20例患者被确诊为心室肌致密化不全患者,有14例患者确诊为左心室受累,有4例患者被确诊为右心室受累。其超声心动图的变现为:①受累腔室扩大,且室壁变得薄弱,运动以及收缩增厚率降低。②右心室心尖部,中间段以及左心室中间段至心尖部的心腔内可见异常粗大的肌小梁以及深陷的隐窝,将受累室壁分为两层,造成致密层较薄,且收缩末期的非致密层和致密层心肌之间的比值≥2.0,儿童≥1.4。③彩色多普勒超声心动图的结果显示,隐窝内血流与心腔相通。结论在诊断和鉴别心肌致密化不全患者时,使用彩色多普勒超声心动图扫查方式具有较好的检测结果。 Objective To investigate the characteristics of ultrasound imaging in patients with myocardial insufficiency, and to investigate the application of color Doppler echocardiography in the diagnosis and differentiation of patients with myocardial insufficiency. Methods Convenient select a total of 38 patients with myocardial insufficiency admitted to our hospital from November 2015 to November 2017 were enrolled. Color Doppler echocardiography was performed on patients with cardiac enlargement and clinical cardiac insufficiency. In this way, the patient's heart structure, blood flow signals and morphology were observed, in particular the endocardium and myocardium of the middle and apical segments of the patient's apex and side walls. Results After color Doppler echocardiography in all patients, 20 patients were diagnosed with ventricular myocardium dysplasia, 14 patients were diagnosed with left ventricular involvement, and 4 patients were diagnosed with right ventricular involvement. The realization of the echocardiogram was as follows: 1.The affected chamber is enlarged, and the wall of the chamber became weak, and the rate of movement and contraction thickening decreased. 2.The right ventricular apex, the middle segment and the middle part of the left ventricle to the apex of the apex can be seen in the abnormal musculature and the deep crypt, which divided the affected wall into two layers, resulting in a thin dense layer, and the ratio between the non-dense layer of the end-systolic phase and the myocardium of the dense layer was ≥ 2.0, and the child ≥ 1.4. 3. The results of color Doppler echocardiography showed that the blood flow in the crypt was connected to the heart chamber. Conclusion Color Doppler echocardiography has good results in the diagnosis and differentiation of patients with myocardial insufficiency.
