儿童期虐待对亚丁湾护航舰员心理健康的影响

Effects of childhood abuse on mental health in naval soldiers escorting in the Aden Gulf

目的研究遭受儿童期虐待的舰员在执行亚丁湾护航的不同阶段心理健康水平的变化规律。方法选取248名执行亚丁湾护航任务的舰员进行追踪研究,运用儿童期受虐问卷(CTQ-SF)、症状自评量表(SCL-90)进行测试对比分析。结果出航前期和出航中期舰员的SCL-90各因子分低于军人常模(P<0.05);出航前期SCL-90躯体化、焦虑因子分高于出航中期(P<0.05);人际关系敏感、忧郁、敌对和偏执因子分低于出航中期(P<0.05)。出航中期情感虐待、躯体虐待、情感忽视和躯体忽视组SCL-90躯体化、人际关系敏感、忧郁和敌对因分高于无虐待组(P<0.05);情感忽视和躯体忽视组SCL-90精神病性因子分高于无虐待组(P<0.05)。出航中期舰员CTQ-SF总分与SCL-90躯体化、人际关系敏感、忧郁、焦虑、敌对、偏执和精神病性呈正相关(P<0.05)。结论有儿童期虐待经历的舰员存在明显的心理健康危机。

Objective To study the change pattern of mental health in naval soldiers with childhood abuse during the escorting in the Aden Gulf. Methods A total of 248 naval soldiers involved in the Aden Gulf escorting were selected and assessed with Childhood Trauma Questionnaire(CTQ) and Symptom Checklist 90(SCL-90). Results All factor scores of SCL-90 in naval soldiers before voyage and in the mid-term of voyage were lower than the national norm for military personnel( P < 0.05). The factor scores of somatization(So) and anxiety(A) of SCL-90 before voyage were significantly higher than those in the mid-term of voyage( P <0.05);and the factor scores of interpersonal sensitivity(IS), depression(D), hostility(H) and paranoid(Pa) of SCL-90 before voyage were significantly lower than those in the mid-term of voyage( P <0.05). In the mid-term of voyage, the factor scores of So, Is, D and H of SCL-90 in naval soldiers with psychological and physical abuse as well as psychological and physical neglect were significantly higher than those in naval soldiers without abuse( P <0.05);and the psychosis(Ps) factor score of SCL-90 in naval soldiers with psychological and physical neglect was significantly higher than that in naval soldiers without abuse( P <0.05). In the mid-term of voyage, the total score of CTQ-SF was positively correlated with factor scores of So, Is, D, A, H, Pa and Ps of SCL-90 in naval soldiers( P <0.05). ConclusionThere exists obvious mental health crisis in naval soldiers with childhood abuse.
