社区轻度认知功能障碍老年人焦虑抑郁情绪及躯体疼痛对睡眠的影响研究

The effect of anxiety,depression and body pain on sleep quality in patients with mild cognitive impairment in community

目的探讨社区轻度认知功能障碍(MCI)老年人焦虑抑郁情绪及躯体疼痛对其睡眠的影响。方法,随访符合MCI诊断的1181名社区老年人,采用一般状况量表、匹茨堡睡眠质量指数量表(PSQI)、17项汉密尔顿抑郁量表(HAMD-17)、Phq-9抑郁量表、Gad-7焦虑量表评估社区老年人的睡眠及抑郁情绪,IDPain量表评估躯体疼痛。结果,回归分析显示,PSQI总分随着焦虑躯体化、HAMD睡眠障碍、HAMD总分、Phq-9抑郁总分、Gad-7焦虑总分、IDPain量表总分增加而增加(P<0.05);但随精神运动性迟缓增加而下降(P<0.05)。PSQI睡眠质量评价也随着HAMD总分、精神运动性迟缓、HAMD睡眠障碍、Phq-9抑郁总分、IDPain量表总分增加而提高一个等级(P<0.05)。结论,对MCI老年人不仅要关注其睡眠质量,更应关注影响睡眠的焦虑抑郁情绪及躯体疼痛等危险因素,从而降低痴呆的发生风险,减轻社会经济负担。

Objective To explore the effects of anxiety, depression and body pain on sleep quality in elderly patients with mild cognitive impairment (MCI) in community. Methods,A total of 1181 elderly patients with MCIin community were selected and assessed withgeneral information questionnaire, Pittsburgh Sleep Quality Index(PSQI), Hamilton Depression Scale-17(HAMD-17), Phq-9, Gad-7 and ID Pain. Results,Regression analysis showed that the total score of PSQI increased with the increases of anxiety somatization score, sleep disorder score in HAMD, and total scores of HAMD, Phq-9,Gad-7, and ID Pain Scale( P<0.05);and the total score of PSQI decreased with the increase of mental retardation score( P<0.05). The sleep quality in PSQI improved with the increases of mental retardation score, sleep disorder score in HAMD and total scores of HAMD, Phq-9 and ID Pain( P<0.05). ConclusionBesides sleep quality, risk factors influencing sleep quality such as anxiety, depression and body pain should also be paid attention to in elderly patients with MCI, so as to reduce the risk of dementia and relieve the social and economic burden.
