摘要
目的研究胰岛细胞分泌功能指标C肽、胰岛素及糖化血红蛋白与儿童糖尿病分型诊断及治疗的相关性及其应用价值。方法将32例儿童糖尿病患儿分为C肽降低组(A组,27例)和非C肽降低组(B组,5例),同时随机选取健康儿童29例作为对照组(C组)。比较治疗前后各组间C肽和胰岛素水平。结果A、B组空腹及餐后0.5、1、2、3h胰岛素、C肽水平与C组比较,A组均低于C组(P<0.05),B组与C组间比较差异无统计学意义(P>0.05)。糖尿病患儿治疗前糖化血红蛋白水平高于健康儿童(P<0.05)。结论 C肽和胰岛素水平可用于儿童糖尿病分型诊断,糖化血红蛋白、胰岛素水平对指导合理治疗和疗效评价有重要意义。
Objective To investigate the relations between serum C peptide,insulin and glycosylated hemoglobin(HbA1c) and the diagnosis and treatment of diabetes mellitus in children. Methods The levels of C-peptide,insulin and HbA1c were measured in 32 children,including 27 cases(group A) with C-peptide increasing and 5 cases(group B) without C-peptide increasing.29 healthy children were randomly selected as control group(group C). Results The levels of HbA1c in both group A and B were higher than those in group C( P <0.05).The levels of insulin and C-peptide in group A were significantly higher than group C(limosis,and 0.5,1,2 and 3 hours after eating respectively, P <0.05),and those of group B were without significant differences compared with group C( P >0.05). Conclusion The levels of C peptide and insulin could be used to guide the classification of diabetes mellitus in children.The levels of HbA1c and insulin could guide the rational treatment of diabetes mellitus in children and evaluation of treatment effects.
