摘要
目的研究IL-32与CCL-18在不同严重程度慢性阻塞性肺疾病(COPD)患者中的表达水平并以分析。方法选取2017年1月1日-2018年12月31日于某院呼吸科接受治疗的稳定期COPD患者40例记为稳定期组,急性加重期COPD(AECOPD)患者40例记为AECOPD组,健康体检人员40例记为对照组。分别比较三组血清IL-32、CCL-18水平,并分析血清IL-32、CCL-18水平与COPD患者病情严重程度的关系,对比病情严重程度不同的CDPD患者CAT、MMRC评分情况,并予以相关性分析。结果对照组、稳定期组、AECOPD组血清IL-32、CCL-18水平呈逐渐升高趋势,且经单因素方差分析可得:各组间对比差异均有统计学意义(均P<0.05)。稳定期组与AECOPD组中Ⅰ级、Ⅱ级、Ⅲ级、Ⅳ级血清IL-32、CCL-18水平均呈逐渐升高趋势,且经单因素方差分析可得:各组间对比差异均有统计学意义(均P<0.05)。稳定期组与AECOPD组中Ⅰ级、Ⅱ级、Ⅲ级、Ⅳ级CAT、MMRC评分均呈逐渐升高趋势,且经单因素方差分析可得:各组间对比差异均有统计学意义(均P<0.05)。经Spreaman相关性分析可得:COPD患者血清IL-32、CCL-18与CAT、MMRC评分均呈正相关关系(均P<0.05)。Ⅰ级、Ⅱ级、Ⅲ级、Ⅳ级AECOPD组患者入院当天血清IL-32、CCL-18水平均高于入院第4天,而上述各组患者入院第4天血清IL-32、CCL-18水平又高于入院第7天,差异均有统计学意义(均P<0.05)。结论 COPD患者血清IL-32、CCL-18存在明显高表达,且随着病情严重程度的逐渐增加,上述两项指标水平随之升高。
Objective To investigate and analyze the expression levels of IL-32 and CCL-18 in patients with chronic obstructive pulmonary disease(COPD) of different severity. Methods From January 2017 to December2018, a hospital 40 patients with COPD for plateau group, 40 patients with acute aggravating period of COPD(AECOPD) for AECOPD group, healthy physical examination personnel 40 cases as control group.Three groups were compared, the serum levels of IL-32, CCL-18 and analyzed serum levels of IL-32, CCL-18 relation with the severity of COPD patients, compared the treatment of patients with different severity of CDPD CAT, MMRC grading, and correlation analysis. Results The control group, the plateau group, the serum levels of IL-32 AECOPD group, CCL-18 levels showed a trend of increasing gradually, and the single factor analysis of variance available: contrast differences between groups were statistically significant(all P<0.05).Stability in the group with AECOPD Ⅰ,Ⅱ,Ⅲ,Ⅳ serum level of IL-32, CCL-18 showed a trend of increasing gradually, and the single factor analysis of variance available: contrast differences between groups were statistically significant(all P<0.05).Stability in the group with AECOPD Ⅰ,Ⅱ,Ⅲ,Ⅳ CAT, MMRC score all showed a trend of increasing gradually, and the single factor analysis of variance available: contrast differences between groups were statistically significant(all P < 0.05).According to Spreaman correlation analysis, serum IL-32 and CCL-18 of COPD patients were positively correlated with CAT and MMRC scores(all P<0.05).Ⅰ,Ⅱ,Ⅲ,Ⅳ AECOPD patients admitted to hospital on the day of the serum IL-32, CCL-18 levels were higher than in hospital 4 d, and these groups of patients admitted to hospital d serum IL-32, CCL 4 to 18 level is higher than 7 d, admission are statistically significant differences(all P<0.05). Conclusion Serum IL-32 and CCL-18 expression in COPD patients were significantly high, and the levels of these two indicators increased with the severity of the disease.
