慢性粒细胞白血病患者规范化管理的效果

Effects of Chronic Myelocytic Leukemia standard management program

目的 :分析慢性粒细胞白血病(Chronic Myelocytic Leukemia,CML)患者规范化管理的方法和效果。方法 :便利选取106例确诊为CML的患者,随机分为干预组和对照组各53例。前者采用以Donabedian的结构-过程-结果三维质量模式为理论框架的患者规范化管理服务;后者采用常规护理。结果 :干预结束时,干预组的服药依从性优于对照组,差异有统计学意义(t=2.628,P=0.010);两组生活质量(QualityofLife,QOL)比较,差异无统计学意义(t=0.008, P=0.993)。干预结束后6个月,干预组的服药依从性和QOL均优于对照组,差异有统计学意义(t=8.117,P<0.001;t=2.572,P=0.012)。结论 :CML患者规范化管理方案能够有效改善患者的服药依从性和健康相关QOL。

Objective: To explore the effects of chronic myelocytic leukemia(CML) standard management program based on structure-process-outcome model. Methods: A total of 106 CML patients were randomly assigned to the standard management program group(n=53) and the control group(n=53). On the basis of Donabedian’s quality of care program of structure-process-outcome, standardized management service was provided to intervention group, while, the control group received general nursing care. Results: After the intervention, the results indicated that there were significant differences in scores of medication compliance between the two groups(t=2.628, P=0.010);there were no significant differences in Quality of Life(QOL) between the two groups(t=0.008, P=0.993). After six months of the intervention, there were significant differences between the two groups in the medication compliance(t=8.117, P<0.001) and QOL(t=2.572, P=0.012)respectively, with higher scores in the intervention group. Conclusion: Standardized CML management program can effectively improve the medication compliance and QOL in patients with CML.
