血透室医务人员手卫生及手机污染情况调查 被引量:11

摘要 目的调查某院血透室医务人员手卫生及手机污染情况,为加强医务人员手卫生,防治院内交叉感染提供依据。方法某院血透室医务人员共58名,随机抽取50人,其中主治医师17名,护理人员33名。连续5天使用琼脂直接接触采样培养法采集医务人员手部细菌,共250例细菌样本;采用自行设计的问卷调查上述研究对象的手机使用情况,同时使用同样的方法采集上述调查对象的手机细菌。将医务人员单位菌落计数高于10cfu/cm^2计为不合格,低于10cfu/cm^2计为合格。结果医务人员洗手前手部细菌合格率为12%,洗手后手部细菌合格率为62.00%,洗手能有效提高医务人员手部细菌合格率,差异具有统计学意义(χ^2=92.92,P=0.00)。手机使用情况调查问卷显示,调查对象中,有90.00%会将手机带入工作中,76.00%会在工作中使用手机,其中13.16%将手机用于私事,21.05%将手机用于公事。94.00%的医务工作者认为手机会引起交叉感染,为手机套保护套的有62.00%,使用手机前洗手和使用手机后洗手的分别有42.00%和36.00%。每天清洁手0次、1~2次及3次以下的人分别为16.00%、64.00%和20.00%。检测到的50例手机中,有45例被细菌污染,菌落数为12~153cfu/cm^2,平均(112±21)cfu/cm^2。其中检测率最高的细菌是铜绿假单胞杆菌,其次是金黄色葡萄球菌,分别为91.07%和85.71%。结论血透室医务人员手部带菌率较高,手卫生需要得到重视。手机作为医疗环境的一个要素,对医院交叉感染有较大的影响,应加强对医务人员手机的管理,提高医务人员意识,自觉做到上班时间不带手机、不使用手机。 Objective To investigate the situation of hand hygiene and mobile phone pollution of medical staff in the hemodialysis room,and to lay the foundation for strengthening hand hygiene and preventing cross infection in the hospital.Methods A total of 58 medical and nursing personnel were recruited from hemodialysis room in our hospital.50 of them were randomly selected including 33 nursing staff and 17 attending doctors.A total of 250 samples of bacteria were collected through direct contact sampling and culture of agar for 5 days.A self-designed questionnaire was used to investigate the mobile phone use of the above participants,and the same method was used to collect the bacteria of the mobile phone.The colony count of medical staff was considered to be unqualified if it was higher than 10cfu/cm^2 and qualified if it was lower than 10cfu/cm^2.Results The qualification rate of hand bacteria before hand washing was 12%,and the passing rate of hand bacteria after hand washing was 62%, and hand washing could effectively improve the qualification rate of hand bacteria of medical staff.The difference was statistically significant (χ^2 =92.92,P =0.00).The mobile phone use questionnaire showed that 90% of the respondents would bring their mobile phones into their workplace,and 76% would use their mobile phones in their work,of which 13.16% used their mobile phones for private affairs and 21.05% of them used their mobile phones for public affairs.94% of health care workers believed that mobile phones would cause cross infection,62% for a handset cover,42% and 36% for washing hands before using handsets and using mobile phones.The proportion of people who cleaned their hands 0 times a day,1~2 times and 3 times a day were 16%,64% and 20%,respectively. Of the 50 mobile phones detected,45 were contaminated by bacteria,with a colony count of 12~153cfu/cm2 and an average of 112±21 cfu/cm2.Among them,the highest detection rate was pseudomonas aeruginosa,followed by staphylococcus aureus,accounting for 91.07% and 85.71% respectively.Conclusion The hand hygiene rate of medical staff in hemodialysis room is high,and hand hygiene should be emphasized.As a key element of medical environment,mobile phone has a great influence on the cross infection of the hospital.It is necessary to strengthen the management of medical staff and improve the consciousness of medical personnel.
