多部门联合干预对ICU尿管相关泌尿系感染预防控制效果研究

Research on the Effect of Multi-Department Combined Intervention on Prevention and Control of Catheter Associated Urinary Tract Infection in ICU

目的探讨研究多部门联合干预对ICU 尿管相关泌尿系感染(CAUTI)预防控制效果,为ICU 降低CAUTI 提供新的思路。方法选取2016 年1 月—2018 年12 月甘肃武威肿瘤医院ICU 留置尿管患者进行CAUTI 监测,共3 412 份,按时间顺序分为对照组和干预组,对比两组患者CAUTI 千日感染率、防控措施落实合格率、病原学标本送检和抗菌药物合格率。结果干预组CAUTI 千日感染率(1.63‰)较对照组(2.86‰)明显下降,防控措施落实合格率、病原学标本送检和抗菌药物合格率明显升高,差异有统计学意义(P<0.05)。结论多部门联合干预可显著降低ICU 患者CAUTI 千日感染率,规范了医护人员各项防控措施的落实,同时也可以促进抗菌药物合理应用及病原学送检,可作为一种新的管理模式运用于临床。

OBjECTIVE To investigate the effect of multi-department combined intervention on prevention and control of Catheter Associated Urinary Tract Infection(CAUTI)in ICU, provide new ideas for reducing CAUTI in ICU. METHODS CAUTI monitoring was performed in ICU indwelling catheter patients from January 2016 to December 2018. A total of 3 412 patients were divided into control group and intervention group in chronological order. The thousand-day infection rates of CAUTI,the acceptance rate of prevention and control measures, the inspection of pathogenic specimens and the acceptance rate of antibacterial drugs were compared between these two groups. RESULTS The infection rate of CAUTI in the intervention group(1.63‰)was significantly lower than that in the control group (2.86‰),the acceptance rate of prevention and control measures,the examination of pathogenic specimens and the acceptance rate of antibacterial drugs were significantly increased in the intervention group(P <0.05). CONCLUSION Multi-department joint intervention could significantly reduce the thousand-day infection rate of CAUTI in ICU,standardize the implementation of various prevention and control measures for medical staff,and at the same time,promote the rational application of antimicrobial agents and the examination of etiology,which could be used as a new management mode in clinical practice.
