点阵铒激光联合羟基乙酸治疗面部痤疮瘢痕临床观察

Clinical Efficacy of Combined Lattice Erbium Laser with Glycolic Acid Treatment of Facial Acne Scar

目的:探讨点阵铒激光联合羟基乙酸治疗面部痤疮瘢痕的临床疗效及安全性。方法:选取2015年3月-2018年8月笔者医院面部痤疮瘢痕患者86例,根据治疗方法分为对照组(n=45,点阵铒激光治疗)和试验组(n=41,点阵铒激光联合羟基乙酸治疗)。比较两组患者临床疗效、治疗前后ECCA评分及舒适度、痂皮脱落时间、误工期。对治疗前后色素斑、红斑、纹理、毛孔、紫质等皮肤特征指标进行定量检测,记录不良反应发生情况。结果:试验组治疗后总有效率为90.24%(37/41),明显高于对照组的68.89%(31/45),差异具有统计学意义(χ^2=5.91,P<0.05)。两组治疗后ECCA评分均明显下降(P<0.05),但试验组下降幅度高于对照组(P<0.05)。试验组患者舒适度高于对照组(P<0.05),痂皮脱落时间、误工期低于对照组(P<0.05)。与治疗前比较,两组治疗后色素斑、红斑、毛孔、纹理、紫质等指标均有明显改善(P<0.05),但试验组色素斑、毛孔、紫质改善程度高于对照组(P<0.05),所有患者均未发生严重不良反应。结论:点阵铒激光联合羟基乙酸治疗痤疮瘢痕的疗效优于单独使用点阵铒激光,安全性较好。

Objective To investigate the effect of Lattice erbium laser combined with glycolic acid in the treatment of facial acne scars. Methods 86 patients with facial acne scars from Fourth People’s Hospital of Langfang City between March2015 and August 2018 were analysed.The patients were divided into control group(n=45) and experimental group(n=41)depending on the treatment. Clinical curative effect, ECCA score before and after treatment, patient comfort, scab peeling time, delays in working hours were compared between the two groups. Skin characteristics such as pigment spot, erythema,texture, pore, purple matter before and after treatment were measured in the two groups. Adverse reactions were recorded.Results The total effective rate of experimental group after treatment(90.24%,37/41) was significantly higher than the control group(68.89%,31/45). The difference was statistically significant(χ~2=5.91,P<0.05). After treatment, ECCA scores of both groups decreased significantly(P<0.05), but the decrease rate of experimental group was higher than that of control group(P<0.05).The comfort level of patients in experimental group was higher than that of control group(P<0.05), and the scab peeling time and the delays in working hours were lower than those of control group(P<0.05).Compared with those before treatment, pigment spots, erythema, pore, texture and purple matter were significantly improved in both groups after treatment(P<0.05), but the improvement degree of pigment spots, pore and purple matter in experimental group was higher than those in control group(P<0.05). No serious adverse reactions were observed in all patients. Conclusion Efficacy of erbium-doped yttrium garnet lasers combined with glycolic acid in the treatment of facial acne scars is better than erbium-doped yttrium garnet lasers alone,and has good safety.
