不同剂量舒芬太尼复合丙泊酚用于宫腔镜手术麻醉效果比较

Anesthetic Effect of Different Dosages of Sufentanil Combined with Propofol in Hysteroscopic Operation:A Comparative Study

目的探讨不同剂量舒芬太尼复合丙泊酚用于宫腔镜手术麻醉的效果。方法选取医院2016年7月至2017年7月收治的宫腔镜手术麻醉患者93例,按随机数字表法分为A组、B组和C组,各31例,分别给予枸橼酸舒芬太尼注射液0. 10μg/kg、0. 15μg/kg、0. 20μg/kg,静脉注射,配合丙泊酚注射液(2 mg/kg)镇静治疗,观察3组患者的收缩压、舒张压、血氧饱和度(SpO2)、术后意识恢复时间、术中丙泊酚用量及不良反应发生情况。结果 3组患者术前、术中、术后的收缩压、舒张压无显著差异(P> 0. 05);术后,A组患者SpO2显著高于B组和C组,意识恢复时间显著短于B组和C组,丙泊酚用量显著少于B组和C组(P <0. 05);3组患者恶心呕吐、头痛等不良反应的发生率无显著差异(P> 0. 05),但C组烦躁不安发生率为22. 58%,明显高于A组的3. 23%和B组的6. 45%(P <0. 05)。结论 0. 10μg/kg舒芬太尼复合丙泊酚用于宫腔镜手术麻醉,可有效稳定患者麻醉期的血压和SpO2,缩短意识恢复时间,降低苏醒期躁动发生率,减少丙泊酚用量,且不增加不良反应。

Objective To investigate the effect of different dosages of sufentanil combined with propofol on anesthesia in the hysteroscopic operation. Methods Totally 93 patients underwent hysteroscopic operation and anesthesia in our hospital from July 2016 to July2017 were selected and divided into group A,B and C by the random number table method,31 cases in each group. The patients in group A,B and C were administered intravenously with 0. 10,0. 15 and 0. 20 μg/kg Sufentanil Citrate Injection respectively,combined with Propofol Injection( 2 mg/kg) for sedation. The systolic blood pressure,diastolic blood pressure,blood oxygen saturation( SpO2),the recovery time of consciousness,propofol dosage and adverse reaction in the 3 groups were observed. Results There was no significant difference in systolic blood pressure and diastolic blood pressure among the 3 groups before,during and after operation( P > 0. 05).After operation,the SpO2 in group A was significantly higher than that in group B and group C,the recovery time of consciousness in group A was significantly shorter than that in group B and group C,and the propofol dosage in group A was significantly lower than that in group B and group C( P < 0. 05). There was no significant difference in the incidence of nausea,vomiting and headache among the 3 groups( P > 0. 05). However,the incidence of restlessness in group C was 22. 58%,which was significantly higher than 3. 23%in group A and 6. 45% in group B( P < 0. 05). Conclusion 0. 10 μg/kg sufentanil combined with propofol for anesthesia in the hysteroscopic operation can effectively stabilize patients’ blood pressure and SPO2,shorten the recovery time of consciousness,reduce the incidence of restlessness during recovery and the dosage of propofol without increasing the adverse reactions.
