莫匹罗星软膏对烧伤感染者创面愈合及炎性因子的影响

Effect of Mopirocin Ointment on Wound Healing and Inflammatory Factors in Patients with Burn Infection

目的探讨莫匹罗星软膏对烧伤感染患者创面愈合速度及炎性因子水平的影响。方法选取医院2016年4月至2018年4月收治的烧伤感染患者102例,按随机数字表法分为对照组和观察组,各51例。两组患者均于入院后使用1∶2 000氯己定清洗、消毒创面,去除腐皮,并根据患者实际病情给予营养支持、消炎镇痛类药物及维持水电解质平衡等基础治疗。在此基础上,对照组患者予磺胺嘧啶银乳膏,观察组患者予莫匹罗星软膏,均治疗22 d。结果治疗4 d后,两组创面回缩率比较,差异无统计学意义(P> 0. 05);治疗10,16,22 d后,两组创面回缩率均明显上升,且观察组明显大于对照组(P <0. 05);观察组患者创面愈合时间明显短于对照组(P <0. 05);治疗后,两组患者肿瘤坏死因子-α(TNF-α)、白细胞介素6(IL-6)水平均明显下降,且观察组明显均低于对照组(P <0. 05);观察组细菌清除率明显大于对照组(P <0. 05);治疗后,两组生活质量各维度评分均明显上升,且观察组明显高于对照组(P <0. 05)。结论莫匹罗星软膏可加快烧伤感染创面愈合,利于减轻炎性反应,提高细菌清除率,改善生活质量。

Objective To investigate the effect of Mupirocin Ointment on wound healing rate and the levels of inflammatory factors in patients with burn infection. Methods Totally 102 patients with burn infection admitted to our hospital from April 2016 to April 2018 were selected and divided into the control group and the observation group according to the random number table method,51 cases in each group. After admission,the patients in the two groups were treated with 1 ∶ 2 000 chlorhexidine to clean and disinfect wounds,remove putrefied skin,and give nutritional support,anti-inflammatory and analgesic drugs and maintain water and electrolyte balance according to the actual condition. On this basis,the patients in the control group were treated with Sulfadiazine Silver-Zinc Cream,while the patients in the observation group were treated with Mupirocin Ointment. Both groups were treated for 22 d. Results After 4 d of treatment,there was no statistically significant difference in wound healing rate between the two groups( P > 0. 05). After 10,16 and22 d of treatment,the wound healing rate of the two groups showed an upward trend,and that in the observation group was higher than that in the control group( P < 0. 05). The wound healing time in the observation group was significantly shorter than that in the control group( P < 0. 05). After treatment,the levels of tumor necrosis factor-α( TNF-α) and interleukin-6( IL-6) in the two groups showed a decreasing trend,and those in the observation group were significantly lower than those in the control group( P < 0. 05). The bacterial clearance rate in the observation group was significantly higher than that in the control group( P < 0. 05). After treatment,the scores of quality of life in all dimensions in the two groups were significantly improved,and those in the observation group were significantly higher than those in the control group( P < 0. 05). Conclusion Mopirocin Ointment can accelerate wound healing of burn infection,alleviate inflammatory reaction,improve bacterial clearance rate and quality of life.
