2014年至2018年北海市免费艾滋病抗病毒药品使用调查与分析

Utilization of Free Antiviral Drugs for Acquired Immune Deficiency Syndrome in Beihai City from 2014 to 2018

目的了解北海市辖区免费艾滋病抗病毒药品(以下简称抗艾药)的使用情况及趋势,分析其用药合理性。方法统计并分析2014年至2018年北海市辖区抗艾药的在治患者数、用药量、用药金额、用药频度(DDDs)、限定日费用(DDC)。结果在治患者数逐年上升,2018年比2014年增长91. 42%;抗艾药用量和DDDs逐年增大,药品费用持续增长,2018年用药金额比2014年增长129. 35%;拉米夫定(3TC)的使用比例为93. 58%~100. 00%,且DDDs最高、DDC最低,是抗艾滋病病毒治疗的骨干药物;富马酸替诺福韦二吡呋酯(TDF)替换齐多夫定(AZT)、依非韦伦(EFV)替换奈韦拉平(NVP)成为抗艾滋病病毒治疗方案的首选药物;洛匹那韦利托那韦(LPV/r)使用比例最小、DDDs最低、DDC最高,是抗艾滋病病毒二线治疗药物。结论北海市辖区2014年至2018年抗艾药使用基本合理、经济、规范。在治患者数的增加是药品费用增长的刚性因素。

Objective To investigate the utilization status and trend of free antiviral drugs for acquired immune deficiency syndrome( AIDS) in Beihai city from 2014 to 2018,and to analyze the rationality of their use. Methods The number of patients treated,dosage,sales amount,defined daily dose( DDDs),defined daily cost( DDC) of free antiviral drugs for AIDS were analyzed in Beihai city from2014 to 2018. Results The number of patients treated increased year by year,increasing by 91. 42% in 2018 compared with that in2014. The use of antiviral drugs and DDDs increased year by year,and the cost of drugs continued to increase,increasing by 129. 35%in 2018 compared with that in 2014. The use ratio of Lamivudine( 3 TC) was 93. 58%-100. 00% with the highest DDDs and the lowest DDC,which was the backbone drug of anti-HIV treatment. Tenofovir disoproxil fumarate( TDF) replaced zidovudine( AZT) and efavirenz( EFV) replaced nevirapine( NVP) as the first choice of first-line antiviral treatment options. Lopinavir and ritonavir( LPV/r)had the lowest use ratio,the lowest DDDs and the highest DDC,which was the second-line anti-HIV drug. Conclusion The utilization of free antiviral drugs for AIDS in Beihai city from 2014 to 2018 is basically reasonable,economical and standardized. The increase in the number of patients treated is a rigid factor in the increase in drug costs.
