摘要
目的研究生长发育迟缓(extrauterine growth restriction,EUGR)早产儿纠正胎龄40周时生长发育迟缓情况及其影响因素。方法选取2014年7月至2017年12月在我院新生儿监护室(Neonatal Intensive Care Unit,NICU)住院治疗且正常出院的90例EUGR早产儿,对EUGR早产儿进行纠正胎龄40周时随访,收集相关临床资料:体重、喂养方式、疾病情况、前白蛋白、血清锌含量等,分析其纠正胎龄40周时生长发育迟缓发生情况及其影响因素。结果 EUGR早产儿在纠正胎龄40周时,生长迟缓发生率为64.4%。单因素分析显示,纠正胎龄40周时生长迟缓组与非生长迟缓组相比较,IUGR、NRDS、BPD、有创呼吸机应用、贫血、出生体重、出院体重、胎龄、住院时间、达完全肠内营养时间、前白蛋白量及喂养方式,具有统计学意义(P<0.05),Logistic回归分析显示,EUGR早产儿纠正胎龄40周时生长发育迟缓的发生与IUGR、达完全肠内营养时间、40周时前白蛋白及喂养方式有显著相关性。结论 EUGR早产儿纠正胎龄40周时生长发育迟缓问题依旧十分严峻。
Objective:To investigate the incidences and related influencing factors of the growth retardation of extrauterine growth restriction (EUGR) in preterm infants at corrected age of forty weeks.Methods: Ninety preterm infants of EUGR who were discharged from July 2014 to December 2017 from the Neonatal Intensive Care Unit were selected for study.Regular longitudinal follow up was conducted at corrected age of forty weeks .Related clinical data,such as weight,feeding pattern,diseases,prealbumin,serum zinc were collected.The incidences and related influencing factors of the growth retardation were analyzed.Results: The incidences of the growth retardation of EUGR in preterm infants were 64.4% at corrected age of forty weeks.The single factor analysis showed that IUGR,NRDS,BPD,utilization of mechanical ventilation,anemia,birth weight,the weight at discharge,maternal age,hospitalization time,total enteral nutrition time,prealbumin and feeding patterns had significant differences between growth retardation group and non- growth retardation group at corrected age of forty weeks ( P <0.05).Logistic regression analysis showed that the growth retardation of EUGR in preterm infants at corrected age of forty weeks had significant correlation with intrauterine growth restriction (IUGR),total enteral nutrition time,albumin and feeding method.Conclusion: The growth retardation at corrected age of forty weeks is extremely serious.
