集束化管理策略在气管切开多重耐药菌管理中的应用效果

Application effect of cluster management strategy in management of multi-drug resistant bacteria in tracheotomy

目的探讨集束化管理策略在气管切开多重耐药菌中的应用效果。方法选取2017年4月至2018年4月气管切开多重耐药菌患者56例为研究对象,按照随机数字表法等分为对照组和观察组,对照组给予常规管理,观察组采用集束化管理策略。比较两组患者感染防控措施依从率、气管切开并发症。结果观察组手卫生执行率、隔离措施正确率、医疗废物处理合格率及医院感染知晓率均高于对照组(P<0.05);观察组气管切开痰痂、出血及误吸发生率均低于对照组(P<0.05);实施集中化管理策略后护理人员隔离标识悬挂床边、速干手消毒剂配置、专用蓝色腕带佩戴、消毒医疗环境及用品、安置病床评分均高于实施前(P<0.05)。结论集束化管理策略用于气管切开多重耐药菌管理中,有助于提高感染防控措施依从率,降低气管切开并发症发生率,值得临床推广应用。

Objective To explore the application effect of cluster management strategy in management of multi-drug resistant bacteria in tracheotomy.Methods To select 56 patients with multi-drug resistant bacteria who underwent tracheotomy from April 2017 to April 2018 as the study subjects.They were divided into control group and observation group equally according to the random number table method.The control group was given routine management,while the observation group was managed by cluster management strategy.The compliance rate of infection prevention and control measures as well as tracheotomy complications were compared between the two groups.Results The rate of hand hygiene,the correct rate of isolation measures,the pass rate of medical waste treatment and the awareness rate of nosocomial infection in the observation group were higher than those of the control group(P<0.05).The incidence of sputum scab in tracheotomy,hemorrhage and aspiration in the observation group were lower than those of the control group(P<0.05).After implementing the cluster management strategy,the hanging isolation logo beside the bed by nursing staff,quick-drying hand disinfectant configuration,special blue wristband wearing,disinfecting medical environment and supplies,and placement bed scores were higher than that before the implementation(P<0.05).Conclusion Applying cluster management strategy in the management of multi-drug resistant bacteria in tracheotomy can improve the compliance rate of infection prevention and control measures and reduce the incidence of tracheotomy complications,which was worthy of clinical promotion and application.
