微创DHS内固定与PFNA内固定治疗老年股骨粗隆间骨折的临床疗效比较

Comparison of clinical efficacy between minimally invasive DHS internal fixation and PFNA internal fixation for the treatment of intertrochanteric fractures in the elderly

目的探讨微创DHS内固定与PFNA内固定治疗老年股骨粗隆间骨折的临床疗效。方法选取2016年10月~2018年1月我院老年股骨粗隆间骨折患者160例。对照组应用微创DHS内固定治疗。研究组应用PFNA内固定治疗。比较两组患者的治疗效果、并发症及手术时间、手术出血量、住院时间、负重时间、愈合时间。结果研究组患者在治疗基本情况方面优于对照组,两组比较差异有统计学意义(P<0.05)。治疗后,研究组患者临床治疗效率为97.50%,高于对照组的77.50%,两组比较差异有统计学意义(P<0.05)。研究组患者Harris评分为(88.57±5.04)分,高于对照组的(82.02±4.68)分(P<0.05);研究组并发症发生率(3.75%)低于对照组(17.50%),两组比较差异有统计学意义(P<0.05)。结论微创DHS内固定和PFNA内固定对老年股骨粗隆间骨折患者进行治疗,均有一定疗效,但综合来看,PFNA内固定效果更佳理想,不仅对机体损伤程度低,而且术后并发症低,患者康复快,可行性强。

Objective To explore the clinical efficacy of minimally invasive DHS internal fixation and PFNA internal fixation for the treatment of intertrochanteric fractures in the elderly.Methods A total of 160 elderly patients with intertrochanteric fractures in our hospital from October 2016 to January 2018 were enrolled.The control group was treated with minimally invasive DHS internal fixation.The study group was treated with PFNA internal fixation.The treatment effect,complications and operation time,surgical bleeding volume,hospitalization time,weight-bearing time,and healing time were compared between the two groups.Results The patients in the study group were superior to the control group in the basic treatment,and the difference between two groups was significant(P<0.05).After treatment,the clinical treatment efficiency of the study group was 97.50%,higher than that of the control group(77.50%).The difference between the two groups was statistically significant(P<0.05).The Harris score of the study group was(88.57±5.04),which was higher than that of the control group(82.02±4.68)(P<0.05).The complication rate of the study group was 3.75%,lower than that of the control group(17.50%),with significant difference(P<0.05).Conclusion Minimally invasive DHS internal fixation and PFNA internal fixation both have a certain effect for the treatment of elderly patients with intertrochanteric fractures.But comprehensively,the effect of PFNA internal fixation is better,which has not only lower damage to the body,but also lower incidence of complications.The patient recovers quickly,and the feasibility is strong.
