妊娠期蛋白尿患者蛋白尿转归与瘦素、膜糖蛋白的相关性分析

Analysis on the correlation between proteinuria outcomes and leptin and membrane glycoprotein in the patients with proteinuria during pregnancy

目的探讨妊娠期蛋白尿患者蛋白尿转归与瘦素、膜糖蛋白的相关性,为妊娠期蛋白尿患者的临床诊治提供参考。方法回顾性分析我院2015年1月~2018年2月收治的4698例孕妇的临床资料,并选取120例资料完整的妊娠期蛋白尿患者作为研究对象,按照蛋白尿转归情况,将其分为转归良好组(n=73)和转归不良组(n=47)两组。比较两组患者的临床资料差异,并采用多因素Logistic回归分析法对妊娠期蛋白尿患者蛋白尿转归不良进行研究,并采用Pearson法对妊娠期蛋白尿患者蛋白尿转归与瘦素、膜糖蛋白的相关性进行分析。结果两组患者在妊娠次数、舒张压、收缩压、血红蛋白、血尿酸、血尿素氮以及丙氨酸转氨酶水平等临床资料差异不显著(P>0.05),在血浆白蛋白、血肌酐、天冬氨酸转氨酶、CD63、CD62、瘦素、蛋白尿出现时间以及蛋白尿严重程度等差异具有统计学意义(P<0.05)。经多因素Logistic回归分析,CD63、CD62、瘦素以及蛋白尿出现时间是妊娠期蛋白尿患者蛋白尿转归不良的独立危险因素(P<0.05)。妊娠期蛋白尿患者蛋白尿转归与瘦素、CD63、CD62水平呈正相关(P<0.05)。结论妊娠期蛋白尿患者蛋白尿转归与瘦素、膜糖蛋白呈正相关,为妊娠期蛋白尿患者早期临床诊治及蛋白尿不良转归预防提供参考,具有一定的临床应用价值。

Objective To investigate the correlation between proteinuria outcomes and leptin and membrane glycoprotein in the patients with proteinuria during pregnancy,so as to provide reference for the clinical diagnosis and treatment of patients with proteinuria during pregnancy.Methods A retrospective analysis was carried out for the clinical data of 4698 pregnant women who were admitted to our hospital from January 2015 to February 2018,and 120 patients with complete data of gestational proteinuria were selected as subjects.According to the outcomes of proteinuria,they were divided into two groups:the good outcome group(n=73)and the poor outcome group(n=47).The clinical data differences between the two groups of patients were compared.Multivariate Logistic regression analysis was used to study the poor proteinuria outcomes in the patients with proteinuria during pregnancy.The Pearson method was used to analyze the correlation between proteinuria outcomes and leptin and membrane glycoprotein in the patients with proteinuria during pregnancy.Results There were no significant differences in clinical data such as times of pregnancy,diastolic blood pressure,systolic blood pressure,hemoglobin,serum uric acid,blood urea nitrogen and alanine aminotransferase levels in both groups(P>0.05).The differences in plasma albumin,serum creatinine,aspartate aminotransferase,CD63,CD62,leptin,occurrence time of proteinuria and degree of proteinuria were statistically significant(P<0.05).After multivariate Logistic regression analysis,CD63,CD62,leptin and occurrence time of proteinuria were independent risk factors for poor proteinuria outcomes in the patients with proteinuria during pregnancy(P<0.05).Proteinuria outcomes were positively correlated with leptin,CD63 and CD62 levels in the patients with proteinuria during pregnancy(P<0.05).Conclusion Proteinuria outcomes in the patients with proteinuria during pregnancy is positively correlated with leptin and membrane glycoprotein.It provides reference for early clinical diagnosis and treatment for the patients with proteinuria during pregnancy and prevention of poor proteinuria outcomes,which has a certain clinical application value.
